This is in reference to herein undersigned’s claim for policy Cash Surrender Value
(CSV) with reference number R-20201218-000384 which, based on the GSIS Support
Services email correspondence dated 18 May 2021 (ANNEX A), is already for check release
and available for pick up at the GSIS Head Office.

In this regard, considering the country’s state of public health emergency due to the
Coronavirus disease (COVID-19) and my untimely transfer to Iloilo City, I would like to
respectfully request for the transmittal of the aforementioned check to the GSIS Iloilo Office
for claiming.

I have attached scanned copies of two (2) valid identification cards for your information
and reference

I am hoping for your kind consideration regarding this request.
