Warm greetings!

I hope you are doing well in the midst of this lingering pandemic. Once again, I am writing your
good Office with a lot of hope that maybe the Office of the President for the second time will be
able to help me raise the necessary payment for my incurred hospital bills in our very own
Philippine General Hospital (PGH) in connection with my confinement for recuperation hereat
after surgical operation in Manila Doctors’ Hospital. I sincerely appreciate the help that you have
provided me for my bills in the aforesaid private hospital.

To date, my incurred hospital bills here in PGH already reached ____________________ Pesos
(PHP __________). I will be forever grateful if the Office of the President through OSAP can
provide me additional financial assistance in my hard time to be able to pay the said bill.
Attached herewith are the needed requirements in relation to my application for an assistance
from your good Office.
