Warm greetings!

I hope all of you are doing well in the midst of this lingering pandemic. I am
writing this letter with a lot of hope that maybe each of the benevolent Members of
the Board of Directors of PCSO will be able to help me raise the necessary
payment for the incurred hospital bills of my mother who was operated and
confined in De La Salle University Medical Center in Dasmarinas, Cavite from
_______________ up to _______________. It was only last night that we were
able to transfer my still recuperating mother to a government hospital in Manila
upon pledging a promissory note in the aforesaid hospital.

My mother is so worried about the hospital bills but I assured her that the matter
will be taken care of and she does not need to worry. However, the hospital bill is
around _______________ and I only have about _______________. I will be so
grateful if the Board can provide me some financial assistance in my hard time to
be able to pay the hospital bill. Attached herewith are the needed requirements in
relation to my application for a hospital bill assistance from your good Offices.

I hope this supplication of mine will merit your favorable consideration. Thank you
