SUPPLEMENTAL AFFIDAVIT OF EXPLANATION

I _____________________, of legal age and presently assigned at

___________________________________________________, Eastern Police District
(EPD), National Capital Region Police Region (NCRPO), after having sworn to an oath
and depose that;

I hereby executing this affidavit in accordance with the memorandum that I

received on February 17, 2021 for Failure to Implement Two Shifts Work schedule.

That the allegations based on the Notice to Submit Compliance and

Supplemental Affidavit of Explanation for the above infractions mentioned-above. In
connection with, the personnel involved were already complied with the said mandatory
shifting scheduled which is from 9:00AM to 9:00PM vice versa. (Annex “1”).

Further, the deployment plan as well of the personnel aligned with the said
mandatory shifting schedule which addendum to the daily detail order and Daily
Attendance Report (DPAR) that published every day for their references. (Annex “2”)

Likewise, On February 12, 2012 immediately after the said above infractions
SWAT Personnel including their members instantaneously followed and complied the
required scheduled following the directives of Regional Director (RD) NCRPO, as being
implemented thoroughly and religiously at heart that can be as overlook in our Daily
After Activity Report (Annex “4”)

Further, the actions on the said infractions was not manifested by any ill-
motive or BAD FAITH but just immerse with the burden on my shoulder as their Officer
In Charge (OIC) who look over their welfare, thus the Court reiterated;

In Development Bank Vs Commission on Audit, the Court reiterated

“Good faith is a state of mind denoting "honesty of intention, and freedom from
knowledge of circumstances which ought to put the holder upon inquiry; an honest
intention to abstain from taking any unconscientious advantage of another, even
through technicalities of law, together with absence of all information, notice, or benefit
or belief of facts which render transaction unconscientious."

That I am not defy any orders nor construing the regulations but I am humbly pray
to please reconsider this matter as I been in police service for years manifested with my
awards and commendations.

Thus, this manifested that I did not intend, clearly violated nor repudiate any
rules and regulations what was happened were fruits of fortuitous event that is beyond
man’s control attested by the presented medical certificate that I am religiously
performing my job well without any whines and not sloppy during my tour of duty.
 Such other relief just and equitable under the circumstances is likewise being
prayed of.
