HEALTH SAFETY AND ENVIRONMENT CHECK LISTS


The following Check list of HSE activities is prepared so as to be used by all supervision team
members during their day to day supervision & inspection activities.

Personal Protective Equipment (PPE).

 All visitors on construction sites, are wearing Personal Protective Equipment (PPE).
 Personal Protective Equipment (PPE) such as safety boots, ear and eye protectors and
helmets are provided to workers.

 A hard hat (safety helmet) is worn whenever working at the site.


 hearing protectors such as earplugs or earmuffs are used while working in high noise
areas to reduce the noise exposure level.
 Safety glasses, safety goggles or face shields are used when performing work that
involves conditions considered hazardous for the eyes such as

o injurious gases, vapours and liquids (acids, caustics, welding)


o dusts, powders, fumes and mists (scaling, light grinding, spot welding)
o thermal and radiation hazards, such as heat, glare, ultra violet, infrared rays (welding,
metal cutting, furnace tending)
o lasers
o Electrical hazards (arcing and sparks).

 Emergency eyewashes are placed in all hazardous areas.


 Breathing protection such as a respirator is used at sites where dust, fumes, mists, and/or
toxic vapours exist such as

o applying toxic or hazardous materials inside tanks, rooms, or other areas where
adequate ventilation does not exist
o During sandblasting, cutting or grinding concrete, or performing other work with
potential exposure to silica.

 Safety gloves are worn for work involving a risk of exposure to chemicals, or when
handling equipment, machinery, hot objects, or other objects causing a mechanical
hazard, such as:

o wire ropes and slings


o chemicals
o hot items
o grinding
o fillings and sharp materials
o lashing equipment.

 Safety footwear appropriate for the hazards of the work and the work site area are worn
by all employees. for example,
o rubber boots when working in or near damaging liquids
o safety-toe boots when moving or rigging heavy objects.
o Rubber soled and anti skid to boots safeguard against slips, trips and falls
o Chemical resistant boots when employees are working with chemicals.

 Protective Work clothing which is clean, undamaged and of the right size is worn by
workers.
o Work clothing is used correctly, bottomed up to the neck, with long sleeves bottomed
down to the fist. Ties or any type of jewellery are not be worn in the work areas.
o Employees are wearing attention vests made of reflective material which are highly
visible in all levels of light.

 All sanitary workers are equipped with overcoats, sanitary gloves, gum boots.

FALL PROTECTION

 For excavations with straight cut sides fall protection is provided with rigid guardrail systems.
Employees working within 1.8 metres of an excavation with unprotected sides or edge are secured to
a lifeline or fall restraint system
 Employees engaged in work activities on low or high slopes roofs with unprotected sides and edges
1.8 metres or more above a lower level are provided fall prevention and protection by using guardrail
systems, personal fall protection systems, safety net systems, or warning lines.
 For employees working on steel structure erections, access to the steel structure is obtained by use
of ladders, aerial lifts, or other approved personnel hoisting devices. Climbing of structural steel
members, such as columns and diagonal braces, should be reported immediately.
 Employees riding in or working from aerial lifts have secured their safety lanyards to the lift basket at
all times.
 Employees riding in or working from hoisting devices, are provided with independent lifeline and a
rope grab. The employee’s lanyard is connected to the rope grab at all times while aloft.

SLIP AND TRIP HAZARDS

 Working floor areas should be checked for the following:

 Working floor areas are not uneven or wet


 There is no oil or other slippery material on floors,
 The floor coverings are unsuitable
 There are no abrupt changes in levels,
 There are no cables, cords or hoses on the floors,
 The work area does not have poor lighting and poor housekeeping.
 Visibility and lighting is adequate,
 hand rails are provided where required
 tread markers or other floor markings are installed.

CIVIL WORKS GENERAL

 If noise is generated by equipment associated with the construction activities, such as in civil work
and transportation.

 Local residents and authorities are notified beforehand about high noise level operations,
if it is found necessary.
 ground vibration that may cause, building damage is conducted at distances greater than
50 metres (150 feet) from residences.

 dust from activities, such as earthmoving operations, crusher operations, wind blow and traffic
movements is mitigated by using adequate equipment for collecting or preventing dust during
the operations, for example, by providing roads with an artificial watering in sensitive areas or
installation of dust collection equipment for crusher operations
 Drainage system is fully functional during heavy rain, is seen to be capable of dispersing heavy
rainfall and there is no scouring
 adequate portable and permanent sanitation facilities are provided at the construction site.
 Contaminated water facilities are separated from rain water drainage.
 Oily water is collected and stored in sludge tanks, and is disposed as per established rules.
 There are no oil or chemical spills due to leakage from construction equipment, motor vehicles,
or storage containers.
 Adequate equipment, such as spill kits, are kept on-site to deal with spills.
 Rehabilitation is implemented as soon as possible after the construction work is finalized.
 Removal of excess and disposed material
 re-vegetation work in the plant area
 relocating excavated soils as naturally as possible.
 Site area is cleared of all loose and unwanted material to allow for adequate working space.

ROAD WORKS GENERAL

 Cut material are temporarily stored along the road side to prevent eroding into the streams and it
is being reused in the road levelling activities.
 Stabilization of road sides quarry and borrow areas by replanting the trees to minimize erosion;
 Rehabilitation works are implemented during the dry season;
 Excavated areas are restored immediately after excavation to limit the exposure of loose
soils, thus minimizing soil erosion;
 Land clearing is limited to only those areas necessary for the road rehabilitation and
upgrading of the project;
 Installation of silt catch basin/ trap and oil and grease interceptor to avoid water pollution;
 Cross drainage works at regular interval in flood prone areas with adequate size to meet flood
requirements specially minor bridges;
 Provision of water supply and sanitation facilities in construction camps; Provision of covered
trucks from top carrying earth to avoid air dust pollution;
 Disposal of solid waste generated from construction activities as construction spoils and
domestic solid waste from house activities
 Tree plantation on side of internal roads specially on steep landscapes to reduce erosion and
accidental risks, etc.
 Site area is cleared of all loose and unwanted material to allow for adequate working space.

ELECTRO-MECHANICAL WORK & MATERIAL STORAGE AREAS GENERAL

 The worksite is kept neat and presentable.


 Excess items or items that are no longer required are disposed of or stored away.
 Work areas are kept free of oil, grease, mud, and slippery materials.
 Equipment and tools are returned to their proper locations after use.

 Corridors are kept clear at all times.


 Cables and wires are properly laid across corridors and walkways.
 Protection safety fences are placed at the corridors when the nature of the job to be performed
requires it, especially if such areas are near access areas for forklifts, handcarts or personnel.
 Areas, where fire-fighting equipment, emergency exits or access to electrical control panels are
located, are kept unobstructed.
 Hosing, wet wiping, wet sweeping or vacuum is used for housekeeping in locations where
hazardous substances are present, for example, dust, mist, aerosol or fume. Dry sweeping
methods are not used in these conditions.
 Paint, detergents and other flammable liquids are kept in a metal sheet locker. No flammable
materials are left close to hot areas.
 Areas are only used for purposes for which they were intended and signposted.
 Traffic in and out from the storage areas does not cause hazards for other employees.
 All doorways and areas of access or escape are clearly identified and kept clear at all times.
 Access for emergency vehicles and equipment is not blocked by stored material.
 Floor areas immediately surrounding workshop machinery are suitably marked out, are
unobstructed, are kept free of dry loose material and are not slippery.

CONSTRUCTION SITE SECURITY

 Site is surrounded by a security fence


 Security guards are on duty 24 hours a day
 All incoming vehicles are registered at the gate when arriving or leaving
 First aid facilities are established at the site
 A notice is erected to prevent outsiders entering into work sites, “NO ENTRY for Outsiders”.
 No Alcoholic drinks and other substances, which may impair judgment in the work area

SAFETY MANAGEMENT

 Site is surrounded by a security fence


 A safety plan is implemented by the contractor & sub contractors
 A Site Safety Committee is established chaired by the Site Manager.
 The safety committee is promoting a safe work environment, pin pointing unsafe conditions or
acts, and assisting employees in implementation of safety-related issues.
 The committee is making recommendations concerning actions required to ensure a safe work
environment.
 Each safety committee meeting is documented,
 Safety induction training has been conducted for all workers and staffs for safety guidelines and
regulations
 Regular emergency drills, are conducted for workers and staff.
 meeting held from time to time on the various aspects of health and safety.

 Fire response procedures are developed and made familiar to all workers
 well-marked designated fire assembly points, fire alarms, and fire extinguishing equipment are
installed at strategic points
 emergency fire exits included on all buildings at the campsite to minimize fire risks.

 Verify facilities are sanitary


 Verify the following are provided at the site

 Toilets
 Washing area
 Work area cleanliness and Hygiene
 Lighting & ventilation
 First Aide Facilities- First aid facilities such as a first aide box is available in case of minor injury
treatment.

RAP (RESETTLEMENT ACTION PLAN)

 Suggested monitoring indicators are outlined below and include (and not limited to):

 Number and place of public consultation meetings held with PAPs(project


affected people) and local authorities in preparation of or during RAP
implementation;
 Number of PAPs effectively compensated and aggregated amount disbursed
compensation (actual versus planned);
 Number of complaints: (total received, total justified; total non-justified)

 Suggested performance/evaluation indicators include:

 Total nature and level of all complaints received, resolved


 Completion of payment within, or after 2 months of estimated completion date
indicated in the RAP implementation plan;
 Revival of affected farming activities within 4 months after the compensation
payment;
 Submission of monitoring reports at the frequency indicated in the M/E
of the RAP implementation report or quarterly.
