GROUP AGREEMENT

Pursuant to this contract, once accepted, the MAJ/KNELL (“Group”) will hold the following
accommodations at the Days Inn Melbourne, Florida (“hotel”).

PROPERTY
The above listed accommodations have been set aside for the MAJ/KNELL (“Group”). Please be
sure to advise your guests to mention your group name when contacting the hotel for
reservations. The above block will remain in effect until September 25th, 2021 after which time
the remainder of non-guaranteed rooms will be released and put back into the hotels inventory.
Any reservations made after the above “cut off” date will revert back to our daily published rate.

Your group’s coordinator can be reached at 321-724-2051 during normal business hours.
Reservations can be made by calling 321-724-2051, option “0”. Your final room count is due on
September 25th, 2021.

TAXES
Group is responsible for all federal and local taxes associated with housing and services
rendered by the Hotel. Group agrees to the prices herein and agrees to pay them in accordance
with the payment terms set forth.

PAYMENT
Credit Card Authorization and Individual Incidentals
The Hotel offers multiple payment options which include all major credit cards, cash, and
checks. Please enter your choice of payment here, if different from above:

CANCELLATION
No damages shall be due for a failure of performance due to Acts of God, war, terrorist acts,
government regulation, riots, and disaster or strikes any one of which make performance
impossible. If, for any reason, the rooms reserved hereunder are not available the Hotel may
substitute other rooms, and Group agrees to accept such substitutions. In no event will the Hotel
be liable for consequential damages of any nature for any reason.

Group agrees to notify Hotel of any changes, rescheduling or potential cancellations.

Entire Group and/or all individuals within group must cancel on/by September
29th, 2021 to avoid cancellation penalty of one night plus tax.

CONDUCT
Group agrees to conduct itself and its representatives in an orderly manner in full
compliance with the applicable laws, regulations and Hotel rules. Copies of which
are available through the Sales Department. Group assumes all responsibility for
the conduct of all persons in attendance on the Hotels property and for any
damages done by Group to any part of the Hotel, its premises, or content. This is to
include unauthorized smoking in Non-Smoking rooms. Should anyone is your
group violate this procedure, your Group will be charged a one- time cleaning fee
of $125.00 per room. All teen groups must have a chaperone designated for each
group of rooms. Chaperones room must be placed in a central area to aid in the
security and safety of the minors on property.

The persons signing the agreement on behalf of the Hotel and the MAJ/KNELL (“Group”) each
warrant that they are authorized to make agreements and to bind their principals to this
agreement. This contract shall be deemed accepted only after it has been signed by a
representative of the group and thereafter signed by a representative of the Hotel. Acceptance
may be made by facsimile transmission and this contract may be executed in one or more
counterparts, each of which when fully executed, shall be deemed to be an original, and all of
which shall be deemed to be the same agreement If this agreement is not signed by both parties
by September 20th, 2021 the contract is no longer valid and the rooms will be released.
