1. Relative to OUA MEMO 00-0721-0129, OUA MEMO 00-721-0169 and OUA

MEMO 00-0821-0254 of the Office of the Undersecretary for Administration this
Office announced the conduct of the Registration of PNPKI Digital Certificates
through the LMS for DepEd teaching and non-teaching personnel not later than
24 September 2021.

2. Attached are the Procedure on How to Access DepEd Learning Management

System Schools Division of City of San Jose del Monte and Procedure on How to
Accomplish the PNPKI Application Form.

PROCEDURES ON HOW TO ACCESS DEPED LEARNING MANAGEMENT SYSTEM

1. Enter https://r3-1.lms.deped.gov.ph/moodle/login/index.php
2. Click DepEd Commons Teachers (also applicable to non-teaching personnel).
3. Choose either Sign in with Google or Sign in with Microsoft.
4. Click Authorize.

5. Click Site Home then click Professional Development.

6. Click the Course for PNPKI.

7. In the Enrolment Options, enter the assigned Enrolment Key. Coordinate with
your School ICT Coordinator.
8. Watch the PNPKI Application Video Instruction for Teacher and How to Fill-
up your PNPKI Application Form. Fill up the Downloaded PNPKI Form.

9. Click Activities San Jose del Monte City.

10. Click PNPKI Questionnaire then click Attempt Quiz Now.

11. Fill up the form (it must be the same information on the PNPKI application.pdf)
type N/A if not applicable.
12. Click Finish attempt...
13. In the PNPKI Questionnaire Summary, scroll down and click Submit all and
finish.

14. Click Upload PNPKI Form Here.

15. In the Submission Status click Add Submission.

16. Click the Files icon or Drag and Drop the PNPKI Application Form.
17. Click Save Changes.

PROCEDURES ON HOW TO ACCOMPLISH THE PNPKI APPLICATION FORM

Requirements:
1. Adobe Acrobat Reader
2. Fillable PNPKI Application Form
3. Electronic copies of passport size photo and signature.

Procedures:
Note: If you have already installed Adobe Acrobat Reader skip Step 1 – 7
1. Open your browser and type the following URL to access the Adobe Acrobat
Reader download page: https://get.adobe.com/reader/
2. Uncheck all optional offers and click the button “Download Acrobat Reader”
3. Wait until the initialization reaches 100% to download the installer.
4. Click Save to download the installer in your computer.
5. Open the folder where the installer is saved and double-click it to run the
setup.
6. Wait until the installer reaches 100%.
7. Click Finish once the installation is complete. Your Adobe Acrobat Reader will
automatically open.
8. Download the PNPKI Application Form. (See Step 8 of Procedures on How
To Access DepDd Learning Management System)
9. Open the folder where you downloaded the PNPKI Application Form. Double-
click to open the file.
10. Type in your necessary details on the appropriate boxes. Red Boxes are
required details that need to be filled out. All entries encoded in the form
should be in BLOCK or CAPITAL LETTERS.
11. Under item 4G (Date of Birth), follow the format provided (DD/MM/YYYY).
You can also use the dropdown to select the date of birth.
12. Under item 4O, type DEPARTMENT OF EDUCATION – SCHOOLS DIVISION
OF CITY OF SAN JOSE DEL MONTE.
13. Under item 4P, type your School ID and Name of School. (eg: 999999 SAMPLE
ELEMENTARY SCHOOL) If school has no School ID yet, only type the name
of the school.
14. Under item 5I, type your DepEd Email Address. School DepEd Email
Addresses are not allowed.
15. Under item 5J, type your alternate email address. If you do not have another
email address, just leave it blank since it is not a required field.

16. Before you attach your picture and electronic signature, take note of the
following:
a. The picture should be taken within the last 6 months.
b. The picture should show the whole head up to shoulders.
c. The picture should have a white background and has the following
requirements:
d. Width = 35mm
e. Height = 45mm
f. File format = .jpg
17. The electronic signature should be clear and have the same file format as the
picture (.jpg). Only black or blue inks. Red is not allowed.
18. Click the placeholder where the photo should be placed (page 1).
19. Click Browse to locate the photo and click open.
20. The Photo is now attached to the fillable form. You can click again the photo
if you wish to change the photo.
21. To attach the signature, go to the declaration page (page 2) and click the
placeholder where the signature should be located.
22. The signature is now attached to the fillable form. You can click again the
signature if you wish to change it.
23. Do the same in the subscriber agreement page (page 4).
24. Once you are done, click File -> Save As…
25. Select the same location from where you opened your PNPKI Application form
by clicking it.
26. Save your PNPKI Application form using the following format:
LASTNAME_FIRSTNAME_EMPLOYEENO_SCHOOLID_CITYOFSANJOSEDELMONTE
27. Congratulations.
