Subject: Construction of site store yard with Major Violations .

This to draw your attention towards the captioned subject that during our site visit to the
construction of new store yard we found :

A. NO PTW , RA , JSP and TBT for the activity .

B. Rental Bobcat was operating at site w/o informed consultants and w/o any
document for the operator .
C. lqamma profession for the operaor is special driver instead of operator .
D. No SEC ID/ S7 For the operator.
E. No SEC ID for ALL Workers in this activity .
F. No Safety officer at site .

Several times we discussed same issues but unfortunately you did not complied
with SEC Safety system .

So that and complying with SEC 5 star HSMS we stopped this activity till to
rectify ALL above violations .
