REPORTING REQUIREMENTS AND FINES

Reporting Requirements

1. Applicable to:
a. Every contractor/permittee/permit holder or
b. Holder of an MPP or its operator
2. The Contractor of a Mineral Agreement or the holder of a Quarry Permit, any of the Sand and
Gravel Permits, Guano Permit, Gemstone Gathering Permit or Small-Scale Mining Permit
a. Required to submit reports (a) to (h), whichever is applicable to its operation
b. Even when there is no production for a given period
3. The report should indicate the causes or reasons why there was no production for the period.
4. Before implementation of any mine suspension or mine closure
a. The Director, Regional Director
b. Governor/Mayor (in the case of quarry resources and Small-Scale Mining Permits)
c. Shall be promptly notified in writing before implementation
5. Submission of the applicable report shall be made part of the terms and conditions of Mineral
Agreements, FTAA, Exploration Permit, Quarry Permit, Sand and Gravel Permit, Guano Permit
and Gemstone Gathering Permit

Fines

ii. failure to pay fines within 1 year

Effect of late or non-submission of reports


1. Ground for the withholding of other pending transactions or the processing of other mining applications
of the Permittee/Contractor/Permit Holder with the Bureau and/or Regional Office
2. Upon due notice

Requirement for Renewal of Application


1. Permittees/Contractors/Permit Holders
2. Secure a written Certification from the P/C Mining Regulatory Board/RD/Director
3. As to the timely and complete submission of the required reports
4. Non-submission of said reports shall be a ground for denial of the renewal application
