RE: Leave of absence from September 21 to October 20, 2021

Dr. Zozobrado,

I am submitting this request for a leave of absence to tend to important personal matters
beginning end of this month. If possible, I would like to take my leave beginning
September 21, 2021 to October 20, 2021 – but I can commit my Saturday and Sundays,
and/or on-call duties if my schedule permits.

I can make myself available intermittently to answer questions via email only
(aqeel.almahdaly@yahoo.com), and I am happy to do whatever necessary to ensure a
smooth transition before my leave begins.

Again, I am possible to take Saturday and Sunday work schedule to help with the
manpower of the nurses’ station, for the entire duration of my leave.
