Dear Sir:

Good day!

I would like to request a three-day leave of absence for family reasons. If possible, I would like to leave
work on June 2, 2014 and return on June 5, 2014. Ill be in Manila on these days to attend our family
reunion. In order to make up for my three days of absence, Ill be on duty on June 8, 12 and 14.
During this time period I would be glad to assist with any questions via email or phone whenever
possible. I will make certain that all of my work is done before leaving on vacation, and will work closely
with you and the other members of the department to make sure that all of my responsibilities are
taken care of during my absence.
