I. Give 5 questions that the nurse might ask to assess each of the
following
1. Abstract Thinking
 “Can you tell me the meaning of the saying “Time is Gold”
 “Can you tell me which of the following is not a food?”
 “Can you interpret the saying “You are the apple of my eye” briefly?”
 “Can you tell me which of the following are plants?”
 “All of the following are furniture, which one is not?”

2. Insight
 “Do you have any problem on the type of medicine me provide?”
 “Do you have any suggestion on how we will handle your situation in ways that is
comfortable for you?”
 “You are taking this medication, can you tell me how this medication affect you?”
 “What do you think is the reason why you are here with us today?”
 “What do you think will be the result of not taking your medication?”

3. Self- Concept
 “If you are able to change one thing about yourself, what would it be?”
 “What characteristic you like the most about yourself?”
 “What part of your body do you like the most?
 “Does your culture affect on how you see yourself?”
 “Do you see yourself beautiful/handsome?”

4. Judgement

 “What will you do if one of your nurse said that she do not want to talk to you?”

 “What will you do if an unknown person ask you to go with him?”

 “Why are you disappointed to yourself?”
 “You are smiling, but you told me that you are sad, are you really feeling sad

 “You told me that you are happy today, but now you seem to be irritated, can you
tell me what happened?”

 “Can you tell me where we are right now?”

 “Can you tell me who is this person standing behind me?”
 “Can you tell me what we are doing right now?”
 “Can you tell me what is your full name?”

II. Case Scenario


The nurse at a mental health clinic is meeting a new client for the first time and plans to
do a psychosocial assessment. When the client arrives, the nurse finds a young woman who
looks somewhat apprehensive and is crying and twisting facial tissues in her hands. The client
can tell the nurse her name and age, but begins crying before she can provide any other
information. The nurse knows it is essential to obtain information from this young woman, but it
is clear she will have trouble answering all interview questions at this time. 
1. How should the nurse approach the crying client? What should the
nurse say and do?

The nurse should take a gentle approach to the client. When the client is able to tell the
nurse why she is crying, the nurse should tell her that he is here and that he is still happy to
listen to her. The nurse should speak in a soothing, rather than authoritative, tone, and sit
beside or in front of the client with an open stance.

2. Identifying five questions the nurse can choose to ask this client initially.
Give a rationale for each.

 “How are you feeling lately?”- a broad opening to encourage the client to express
herself verbally
 “Is there something you would like to share to me?” - a broad opening to encourage
the client to express herself verbally
 “Can you tell me the reason why are you crying?”- encouraging the client to
communicate her feelings and the reasons for her behavior via explanation
 “Are you ready to talk to me or you still need time?”- If the patient still refuses to
share her feelings, the nurse will provide her with room and privacy..
 “Are you anxious right now?”- observing and clarifying the client's actions in order to
decide whether the behavior correlates to what the client would say.

3. What, if any, assumptions might the nurse make about this client and
her situation?
Since the client is a new patient at the mental health facility, the nurse may
assume that she is extremely nervous. Nevertheless, the atmosphere she is experiencing
may have an effect on her stress level. Often, the individual may be experiencing
separation anxiety or be nervous while speaking with a health care professional.

4. If the client decided to leave the clinic before the assessment formally
began, what would the nurse need to do? 

The nurse can inform the client that he will be available at all times and ready to
listen to anything she has to say. Another thing he should do is mention it to the mental
health clinic's other health workers and better record what happened.
