WITHDRAWING MEDICATION FROM A VIAL
1 Check the physician’s order.
2 Wash hands. Prepare the equipment needed.
Check the label of medication to be withdrawn with medication record or cards,

using the principles of the 10 R’s of drug administration.
4 Perform dosage calculations if needed.
Remove the metal or plastic cap from the top of vial, without touching the rubber
5
stopper.
6 Swab the rubber top with alcohol swab.
Remove the cap from the needle by pulling it straight off. Draw back an amount of
7
air into the syringe that is equal to the specific dose of medication to be withdrawn.
Pierce the rubber stopper in the center with the needle tip and inject measured air
8 into the space above the solution. (DO NOT INJECT AIR INTO THE SOLUTION).
The vial may be positioned upright on a flat surface or inverted.
With non-dominant hand, Invert the vial, keeping needle inserted; control syringe

with dominant hand and keep plunger down with thumb.
Pull/ withdraw the needle tip slightly so that the bevel is below the fluid level and
1 aspirate the appropriate amount of solution, while holding the syringe at eye level
0 and vertically. BE CAREFUL NOT TO TOUCH THE BODY (INNER PART) OF THE
PLUNGER.
If air bubbles enter syringe, gently flick syringe barrel with fingers of dominant hand

while keeping a finger on end of plunger; continue holding vial with non-dominant
1
hand.
1 Move the needle past the fluid into the air space, Push plunger in until air is out of
2 syringe. Withdraw additional solution if needed.
After the correct dose is withdrawn, remove the needle from the vial and carefully
1 replace cap over the needle. If a filter/ aspirating needle has been used to draw up
3 the medication and the medication needs to be administered through a needle,
remove the filter needle and replace it with a new needle.
1 Recheck amount of solution in syringe, comparing with drug volume required.
4 Compare drug label with medication record or card.
1 If a multi-dose vial is being used; label the vial with the date and time opened, and
5 store the vial containing the remaining medication according to agency policy.

Discard or restore all equipment appropriately. Perform hand hygiene.

Encircle wheal with black or blue pen noting medication and time.

2 Observe client’s reaction to injection for at least 30 minutes or as ordered. Makes

Insert needle at 90° angle.

Aspirate by pulling back gently on the plunger of the syringe to determine whether
4 the needle is in a blood vessel. Withdraw needle if blood appears and discard the
1 medication, syringe & needle. Prepare a new sterile set-up and inject at another
site.

If NO blood is aspirated, inject medication slowly. Needle withdrawn quickly.

Apply gentle pressure at the site with small, dry cotton.

Observe client’s reaction to injection and makes client comfortable.
