RELEASE WAIVER AND QUITCLAIM

 
KNOW ALL MEN BY THESE PRESENTS:

I, SG. RODEL LIGUAN PUGAO, Filipino, of legal age, a resident of Citio Public
Street Barangay Tamborong San Lorenzo Guimaras, after having been duly sworn in accordance
to law, do hereby depose and state:

1. I was employed as a security guard at Apeiros Security Solutions and Investigations

Agency, Inc. since February 1, 2021;

2. My employment with the company was terminated on August 15, 2021 due to
(voluntary resignation);

3. In consideration of the sum of Seven Thousand Five Hundred Thirty Three &
33/100 (P7,533.33) Philippine Currency duly received by me on October 4, 2021, from
Apeiros Security Solutions and Investigations Agency Inc., representing full
payment of all enumeration and benefits due me under the Labor Code, Social Security Act and
all other labor laws as former employee of the latter or under any existing agreement with respect
thereto including any and all claims of whatever kind and nature which I have or may have
against the latter, I hereby quitclaim, release, discharge and waive any and all actions of
whatsoever nature, whether past, present or contingent, which I may have against Apeiros
Security Solutions and Investigations Agency Inc, its directors, officers, employees, agents and
clients by reason of or arising from my former employment with the latter.

4. Further, I will institute no action, whether civil, criminal, labor or administrative

against Apeiros Security Solutions and Investigations Agency Inc, its directors, officers,
employee’s, agents and clients. Any and all actions which I may have commenced either solely
in my name or jointly with others before any office, board, bureau, court, or tribunal against
Apeiros Security Solutions and Investigation Agency Inc, its directors, employees, agents and
clients are hereby deemed and considered voluntary withdrawn by mean I will no longer testify
or continue to prosecute said action(s).

5. I have read this document and that the contents of these documents, although prepared
in English, was interpreted and explained to me in Filipino by the undersigned personnel officer
and the same is therefore understood by me.

6. I am executing this instrument freely, voluntarily and willingly without any moral or
physical threat, coercion or promise of reward in any form.
