1. The undersigned PNCO is respectfully requesting for the approval of his reassignment from Sub-Station 8
Taguig City Police Station, SPD to Region IV-A , Alaminos Laguna CALABARZON for the following reasons:

a. To provide my family immediate care and assistance because my wife who is a working mom with a
one (1) year and four (4) months old baby is suffering from a heart ailment. She is alone and there is
no one who can help her to look after our child. (Attached herewith are the medical certificates of my
spouse)

b. I also travel almost every day on my motorcycle for about three hours to my work. I have been
travelling from Manila to Laguna for almost five (5) years.

c. To be able to fulfill my obligations to my family and to the PNP at the same time.

2. The undersigned served happily with the PNP, however the situation to this date from which the family in
the job obligations necessitates to reassigned in areas near the domicile to be able to fulfill it.
I also have no pending case/obligations.

3. It is a great privilege to work in the PNP and the undersigned is looking forward to more productive years
in the call of service. Rest assured that the undersigned shall undertake the highest degree of
professionalism and satisfactory performance in all duties and responsibilities.

4. It would be beyond honor if this request be granted favorable consideration from your kindest end.
