That we, JESSE REY GONZALES AUTOR, married to Flordeliza Tumang Autor, AND JOY
AGNES AUTOR RAFANAN, married to Bernardino B. Rafanan Jr., both of legal ages, Filipinos,
residents and with postal address at Champaca St., San Vicente, Tarlac City, do hereby NAME,
APPOINT and CONSTITUTE, _________________________________________________, of legal
age, Filipino, resident of and with postal address at
____________________________________________________, to be our true and lawful ATTORNEY-
IN-FACT for us and in our name, place and stead, for our own use and benefit, to do and perform all or
any of the following acts and things, pertaining to the transfer of ownership or title covered by TCT No.
148605, situated in San Vicente, Tarlac City, and TCT No. 174786, situated in Calayaan, Gerona, Tarlac,
to wit:

1. To transact to all government offices such as BIR, Municipal Assessor’s Office, Provincial
Treasury Office, DAR Office and Registry of Deeds, and other offices pertaining to the
transfer of my torrent title.
2. To file application, submit requirements, sign and receive the Original Documents relative to
the transfer of ownership.
3. To get or obtain required documents by the BIR such as Certified True Copy of Title and Tax
Declaration, Tax Clearance, Landholdings, Death Certificate, Birth Certificate, and Marriage
Certificate/Contract from the Local Civil Registrar; and by other government offices relative
to the transfer of ownership.
4. Generally, to do and perform all and every act and thing which may be necessary in the
exercise of the foregoing powers.

HEREBY GIVING and GRANTING unto our said Attorney-in-fact full power and
authority to do whatever necessary act and things to be done in and about the premises as
fully to all intents and purposes as I might or could do if personally present, and he reby
RATIFYING and CONFIRMING all that my said Attorney-in-fact shall lawfully do or
cause to be done by virtue of these presents.
