Immediate Care of the Newborn Procedure

Objectives:

1. To ensure an airway & maintain respiration.


2. To prevent cold stress (hypothermia).
3. To provide a time for complete observation.
4. To stimulate circulation as adequate to maintain health.
5. To keep the skin of the baby clean & in good condition

Prepare the equipment:

1. Radiant warmer and warm receiving blankets


2. Oxygen suction with sterile catheter
3. Oxygen mask with three different sizes.
4. Laryngoscope with different sizes blades OR airway.
5. Endo- tracheal tubes with different sizes and satellite.
6. Sterile gloves
7. Cord care tray.
a. Sterile scissor.
b.Sterile clamp.
c. Dry cotton and gauze
d.Cotton with alcohol.
8. Vitamin K.
9. Neonatal health card.
10. Baby scale, measuring tape

11. Baby clothes.

Procedure

1. Wash hands and wear gloves.


2. As the head is delivered the mucus and fluid are wiped from the infant’s nose and mouth
with the use of dry gauze to avoid the baby to grasp and aspirate fluid with 1st breath.
3. As soon as the infant is born receive him in a clean blanket or towel to maintain warmth
and to prevent heat loss and hypothermia.
4. Transfer the baby to the heater.

5. Use the small rubber suction catheter which is attached to a mechanical suction to remove
aspiration fluids that may be obstructing the air way.
 Suction is performed through oropharynx.
 Maintain a modified trendelinburg position to facilitate drainage of secretions
 Suction the newborn’s mouth then nose.
6. Provide warmth by keeping the baby dry (start to dry the head, face and the rest of the
baby by dry towel) to minimize loss of infant’s body heat
 Be sure that the baby is dry from any amniotic fluid and avoid air drafts in the room.

7. APGAR Score: Assess the newborn’s condition immediately after birth (at 1 minute)
through Apgar score (heart rate, respiratory effort, muscle tone, reflexes and color) and is
repeated at 5th minutes Apgar score is an index that provides simple accurate and safe
means of quickly appraising the baby’s condition.
8. During the interval following the 1st minute Apgar score, you should continue to evaluate
the infant to determine the normal weight and height and identify any abnormalities.

9. Cord care as the following:
 The cord is clamped or ligated (the first ligature at 3-5 cm, the second ligature at 2
cm, and the cord is cut and be sure that there are small two arteries and one big vein
 Clean umbilical cord and its base with a cotton ball or cotton tipped applicator
moistened with 70% alcohol

10. Identification
 Provide identification of the infant by applying ID band to the arm, which includes
mother’s name, hospital number, newborn sex, time and date of birth
 Take the newborn’s foot prints according to the hospital policy.

11. Eye care using antibacterial agent. i-e- erythromycin to protect the eyes against
infections.

12. Vitamin K administration: A single dose of vitamin K is administered intramuscularly
as a preventive measure against neonatal hemorrhagic disease.

13. Anthropometric measurements:


 Measure head, chest- circumference, length and breast nodules to detect if the baby
appropriate for gestational age or not using measuring tapes.
 H C= 33-35 cm CC= 30-33 cm length= 45-55 cm

14. Newborn assessment include the following:
 Examine the head for normal shape, size and circumference for early detection of any
birth trauma, or congenital anomalies) caput succedaneum, cephalohaematoma or forceps
marks.
 Inspect anterior and posterior fontanels for bulging or sunkenness
 Examine face and neck for congenital anomalies as cleft lips or cleft palate, neck length,
webbing mobility, nose symmetry, septum, and patency & flaring of nostrils.
 Check for respiratory rate, breath sounds, heart sounds, heart sounds rhythm and rate
through auscultation of the chest to ascertain proper position of the heart and normal air
exchange.
 Put the stethoscope on Apex for Apical Pulse
 Assess the skin for scaling, undue wrinkling or birthmarks, lanugo, vernix, texture or
rash, color to indicate the maturity of the baby and exclude any jaundice or abnormalities.
 The abdomen is palpated for early detection of any mass, enlargement, congenital hernia,
swollen organ (liver, spleen, kidney)
 Examine the appropriateness of sex:
o The genitalia for male urethral opening, scrotum and tests.
o Female labia and discharge.
o Insert the thin catheter into the anus to check patency or thermometer when you
take infant’s body temperature
 Check the extremities, shoulders movements, abnormalities of hands, feet, and extra
fingers.

15. Use baby scale for weighing the baby. Normal weight from 2.5 – 4.0 Kg.

16. Provide suitable dress.


17. Take the newborn away from the heater.
18. Wash hands
19. Record your findings into the proper card
20. Clean and remove all equipment
