Accomplishment Report
For the month of September, 2021

Objective:
1. Provide 100% submission of reports required.
2. Provide feedback to 100 % teachers during the IPCRF validating and checking.
3. Implement school programs and projects in the required period of time.
4. Established support to community for different projects and activity.
5. Attended seminar/workshop for teachers and school head.
6. Monitor cleanliness of the school.
7. Supervised the construction of physical facilities.

I. Instructional/Teaching-Learning Process
1. Initiate IPR checking, validating and rating of teacher’s portfolio using e-
COT tool to teachers who performed who their best throughout the year on
September 1,2021
2. Class orientation of Grade 12 students on grading system in the handled
subjects, learning delivery modality, classroom rules and PFA to students
las September 26,2021.
3. Creation of subject Facebook page, group chat etc. for class announcement and other
matters related to the subject.
4. Module sorting and printing in preparation for class distribution in all subjects handled.
5. While on quarantine preparation of module distribution, making SHS class program and
SDRRM work plan submitted last September 30,2021.
II. Human Resource
1. Attended and participate the INSET 2021 last August 30-September
3,2021.
2. Attended and participated Oplan Balik Eskwela on National School
Opening Day “Oplan Kalampag” 2021” on September 13,2021.
 3. Attended and participated the 3rd quarter NSED last September 16,2021.

3.Attended the send-off and welcome ceremony in the installation of the new PSDS in
Pasacao Di
4. Attended the National Quality Management System (NQMS) last
September 20,2021.
5. Initiated asynchronous meeting in preparation of modular distribution on
September 13,2021.
6. Technical assistance extended by PSDS Mary Angieley M. Peñaflor
,Sonny Taugan and Ms. Lalaine V. Fabricante through coaching and
mentoring to T.L.E and Filipino teachers.
III. Learning Environment

1. Attended and facilitate the conduct of remote Brigada Eskwela 2021-2022 campaign by
Barangay in collaboration with the Barangay Officials .
2. Conducted regular disinfection and sanitation in school every Friday for the month of
September 2021.
1. Improvement of SHS Science laboratory, library and others.
2. Due to increase of Covid-19 positive among the 4 teaching and non-teaching staff the
school coordinated the Pasacao MDRRMO and RHU to conduct school sanitation and
disinfection in school last September 10,2021.
IV. Community Involvement
1. Facilitate and attend the conduct of the following;
a. Collaborated with the Barangay in the crafting of BDRRM and
SDRRM plan for three years from September 1-30,2021 every
Saturday and Sunday.
b. Served as co-author, lay-out artist and consultant of BDRRM plan in
Brgy. Bahay, Pasacao Camarines Sur.
c. Served as volunteer during the vaccination drive of people in Pascao, Camarines Sur.

d. Served as Deped volunteer in Sipocot, Camarines Sur vaccination drive.
