Introduction (2 mins)

Last session you assessed yourself by answering the questions in the quiz that I prepared.
Let’s continue our lesson. In this module we are going to discuss the basic types of investigation report.
The basic types of investigation reports are spot report, progress report, after operation report and final
report. Let’s begin by reading the learning targets. The learning targets will serve as your guide and
goal in studying this lesson.

Activity 2: Content Notes (13 mins)

Basic Types of Investigation Report

The following are the basic types of Investigation Report:


1. Spot Report is an immediate initial investigative or incident report addressed to Higher
Headquarters pertaining to the commission of the crime, occurrence of natural or man-made
disaster or unusual incidents involving loss of lives and damage to properties. Must be acted
within 24 hours. When written, a spot report may use the PNP message form, especially if the
reporting officer is far from the receiver. If the reporter is within a particular installation only, the
report may use the Subject- to Letter form.
2. A Progress Report is an accounting of the actions or series of actions undertaken in relation to
an ongoing investigation of a case.
3. After Operation Report is a report that may be rendered after any successful police operation
that leads to the arrest of any member or some members of a syndicated crime group.
4. Final Report is a thorough, in-depth and lengthy account regarding an investigation into an
incident or case as mandated by higher authorities to establish a determination of the truth
and/or how far it could be determined based on the facts and circumstances with the appropriate
recommendation for the proper course/s of action/s to be made.

SUBJECT : Spot Report re Alleged Shooting Incident

DATE : August 24, 2020

1. On August 24, 2020 at about 5:00 o’clock in the morning, Marian Cruz Nicolas, Filipino,
35 years of age (Jan 10, 1985, Cabanatuan city), female, married, college level,
housewife, with contact number 0919-859-322 and a resident of 39 Sangitan Este,
Cabanatuan City, Nueva Ecija called Police Station 4, Bisig, Cabanatuan City, Nueva
Ecija reported the alleged shooting incident that transpired at about 4:30 o’clock in the
morning of August 24, 2020 at Barangay Bitas, Cabanatuan City, Nueva Ecija.
According to the report, of Marian Cruz Nicolas, the victim identified as Matin Lopez Dela
Cruz, Filipino, 45 years of age (Jan 5, 1975, Cabanatuan city), male, married, college
level, driver and a resident of purok 3, #45, Barangay Bitas, Cabanatuan City, Nueva
Ecija was shot by the unidentified suspect riding a motorcycle without plate number at
about 5’6’’ in height while washing his car in front of his house. Suspect immediately left
after the incident. The victim sustained multiple gunshot wounds in the different parts of
his body and was rushed to Good Samaritan Hospital, Cabanatuan City, Nueva Ecija for
medical treatment but was pronounced dead on arrival by the attending physician
thereat. The cadaver of the victim was moved to the Regional Laboratory Office for an
autopsy examination.

2. The motive of the incident was undetermined and the case is still under investigation.
