ACADEMIC INTEGRITY
CONTRACT
Upon enrolling in any of the programs of the Institute of Physical Education, I agree to
behave in accordance with the provisions of the Don Honorio Ventura State University’s academic
policies. The Student Handbook 2019 specifies the rules and regulations pertaining to the
observance of academic integrity and stipulates therein procedures and corresponding penalties.

I have enrolled in this course and I am aware that I have the responsibility to comply with
all the conditions written in this contract. Therefore, I agree to:
1. Be courteous and respectful in all electronic mails, chat communications and online
discussions.
2. Be self-motivated and self-disciplined.
3. Be a good time manager. Always be in advance than merely submitting on time.
4. Be honest and will not indulge to any kind of cheating especially during exams,
quizzes and assignments and other requirements
5. Assume a leadership role and be a teacher when necessary. Voluntarily help my
classmates, bearing in mind, however, that doing my classmates’ work for them is
equivalent to cheating.
6. Develop the needed technology skills.
7. Be active to course discussions. Listen to each other and respond respectfully to
their comments.
8. Cooperate to group and class activities and respect the ideas of others.
9. Comply with all the course policies.
10. Be honest and will not plagiarize any word, phrase, or material to be submitted. Be
aware that falsification of documents, declaration of false information is strictly
prohibited.

By submitting this form, I have fully read and understand my responsibilities as a student
of the Institute of Physical Education in maintaining the academic integrity and pledge not to
engage in any form of academic dishonesty. I also understand that I will be a subject of
disciplinary actions in the conduct of any of the prohibited acts.
