I, DEBBIE S. MEJIAS, Filipino, 31 years old, single, and a resident of

North Mapang, Rizal, Zamboanga del Norte under oath, hereby depose and say:

That I am the mother of DAVE FRANCIS S. MEJIAS, 12 years old, grade 7

at Colegio de San Francisco Javier who is applying for scholarship;

That one of the requirements is a Certificate of No Income which is true to

my case because I am still supported by my parents who is currently a 3 rd year
College student, and I have no income of my own;

That I am executing this affidavit in good faith in order to attest to the facts
above-mentioned and for whatever legal purpose it may serve.
