AFFIDAVIT OF EXPLANATION

I, HANNA CAMILLE M. ELCAMEL, Filipino, 14 years old, single, and resident

of Sibuloc, Sibutad, Zamboanga del Norte, after having been sworn to an oath in
accordance with law, do hereby declare:

That I am an ESC scholar, enrolled in Colegio de San Francisco Javier;

That I would like to transfer to Sibutad National High School due to the
following reasons:
1. Distance constraints because our family transferred residence from
Rizalina, Rizal, Zamboanga del Norte to Sibuloc, Sibutad,
Zamboanga del Norte;

2. My father lost his job which resulted to our financial difficulty.

That my parents deemed it best for me to transfer to a school that is nearer to our
residence now;

I am executing this sworn affidavit in good faith, freely and voluntarily and to
attest to the truthfulness of the foregoing statement, for whatever purpose it may
serve;
