AFFIDAVIT OF DENIAL
I, JUANITO YU LIM, married, of legal age, Filipino citizen, residing at Sta.
Filomena, Dipolog City, after having been duly sworn to in accordance with law do
hereby depose and say:

That I applied for BID Clearance;

That I have never been charged, accused, nor convicted of any crime involving
moral turpitude in any Courts of Justice in the Philippines, nor had I been a subject of
indictment for violation of any ordinance in Manila or elsewhere;

That I am not the same JUANITO LIM (without middle name) accused in the
following criminal/civil cases:

Criminal Cases Nos. : 6257 and 6262

For : Violation of B.P. 22
Place : Batangas

That said person/accused/defendant in the above-mentioned criminal case/civil

case/cases, REFERS TO ANOTHER INDIVIDUAL who by coincidence bears a name
similar to mine;

That I agree to be liable or charged with perjury and/or falsification of public

document if in case the above-mentioned statements are found not to be true;

That I am executing this affidavit in order to attest to the truth of the foregoing
facts/statements/ to clear my name of such cases for the release of my clearance and for
whatever other legal intents and purposes this affidavit may serve.
