All routes will begin and end at the Riverfront Museum Park. Each rider will receive a t-shirt and
enjoy food and entertainment at the post-ride party! Plus, earn team and pledge prizes!
How to register:
1. Mail to BART, 711 N. Main St., Rockford, IL 61103
2. In-Person at any of the four arts organizations
3. Day of Ride at Riverfront Museum Park
4. Online (coming soon!)
Fees:
g Family Ride: $15 for Youth under 12 ($20 day of ride)
g Family Ride: $35 for Adults ($40 day of ride)
g 35-mile and 64-mile rides: $35 for ALL ($40 day of ride)

Other ways to participate:


1. Invite other riders to join you! Create a team and your organization’s logo or team name
will be custom printed on the back of each team rider’s t-shirt.
2. Collect pledges to raise even more money and win prizes!
3. Be a “Friend of the Ride” (non-rider) and still receive a t-shirt!
BART will raise funds and awareness for the Rockford Symphony Orchestra; Rockford Dance
Company; Mendelssohn Performing Arts Center; and Kantorei, the Singing Boys of Rockford,
while also promoting community health and togetherness.

2011 BIKE FOR THE ARTS (BART) PARTICIPANT WAIVER AND RELEASE OF LIABILITY
Waiver and Release of Liability: I acknowledge that this event is an athletic event and is an extreme test of a person’s physical and mental limits that carries with it the potential for death,
serious injury and property loss. The risks include, but are not limited to, those caused by terrain, condition of public roads, facilities, temperature, weather, physical fitness and health of
participants, equipment, vehicular traffic, lack of hydration, and the actions of other people including, but not limited to, participants, volunteers, spectators, event officials, event monitors, and/or
producers of the event. These risks are present for both participants and volunteers. I understand that a bike helmet is required and must be worn at all times while biking in this event. I assume
all of the risks of participating and/or volunteering in this event. I certify that I am physically fit, have sufficiently trained for participation in the event and have not been advised otherwise by a
qualified medical person. I hereby assume liability for any and all medical expenses incurred as a result of training for and/or participation in Bike for the Arts (BART) including, but not limited to,
ambulance transport, hospital stays, physician and medical services, and treatments. I hereby consent to emergency treatment in the event of my injury or illness. In consideration for being
permitted to participate in Bike for the Arts (BART), I, on behalf of myself and my heirs, personal representatives or assigns, waive, release and discharge from any and all liability for my death,
disability, personal injury, property damage, property theft or actions of any kind which may arise from my participation in Bike for the Arts (BART) THE FOLLOWING ENTITIES OR PERSONS:
Bike for the Arts (BART), Rockford Symphony Orchestra, Rockford Dance Company, Mendelssohn Performing Arts Center, Kantorei, the Singing Boys of Rockford, State of Illinois, Department
of Transportation, and their respective officers, agents, employees and members for all suits or actions that may result from any activity by the permittee, its officers, subcontractors, agents or
employees, the directors, officers, employees, volunteers, representatives, and agents of each of the previously mentioned entities, the event holders, event sponsors, event directors, and
event volunteers. I realize that liability may arise from negligence or carelessness on the part of the persons or entities being released and from dangerous or defective equipment or property
owned, maintained or controlled by them or because of their possible liability without fault. Indemnification/Hold Harmless: I, on behalf of myself and my heirs, personal representatives or
assigns, indemnify and hold harmless each of the entities or persons mentioned in the previous paragraph from any and all liabilities or claims made by other individuals or entities as a result of
any of my actions during this event and/or my violation of any term of this Waiver and Release of Liability. Permission to Use Photographs and Other Recordings: I understand that at this event
or related activities, I may be photographed, filmed and/or recorded. I agree to allow my photograph, videotape, motion picture recording, voice or likeness to be used for any legitimate
purpose, including pre-race and post-race publicity, by the event holders, producers, sponsors, organizers and or their respective assigns.
This Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I hereby certify that I
have read this document and I understand its content.

PARENT/GUARDIAN WAIVER FOR MINORS UNDER 18 YEARS OLD


If I register children under the age of 18, I represent and warrant that I am the parent or legal guardian of the minor children or child and have the legal authority to sign this Waiver and Release of
Liability on their behalf. I agree that by proceeding with registration for this event, the terms of this Waiver and Release of Liability shall apply equally to myself and the minor children I have registered.

Tips for Team Captains:


g Anyone can create a team! Some team options include: companies, professional firms, banks,
restaurants, universities, colleges, clubs, churches, volunteer organizations, schools, Girl Scouts, Boy
Scouts, youth clubs, performing arts groups, athletic teams, or just a group of friends.
g Your black and white team logo will appear on the back of each team rider’s T-shirt. So, the more
riders, the more exposure for your company!
g All of the riders do not need to ride the same route. In fact, they don’t have to ride at all! “Friends of
Ride” are welcome as team members.
g Find out if your company has a match program for rider fees and/or pledges. Ask your company to
underwrite a portion or all of the individual rider fees.
g Contact your Human Resources department. Inquire about support through your company’s Wellness
Program.
