QUESTIONNAIRE ON READING LEVEL OF STUDENTS: CHALLENGES AND


ACTION PLAN

This questionnaire seeks to determine the responses of the key informants


on my study READING LEVEL OF STUDENTS: CHALLENGES AND ACTION
PLAN. The study is part of the researcher’s thesis in Master of Arts in Education
and should help teachers, reading coordinators, school heads and Education
Program Supervisor in English assess and review the challenges encountered by
teachers and students in PHIL-IRI and other reading activities to develop
appropriate action plan and interventions to help learners improve their reading
level.

You may or may not write your name as respondent of this study. There
are no right or wrong answers. The researcher is only interested to gather data
based on your perception on the focus of the study. All data and information
generated from this study will be treated as strictly private and confidential. You
are therefore requested to complete the questionnaire as honest as possible.

PART II: CHALLENGES ENCOUNTERED RELATIVE TO THE READING
LEVEL OF STUDENTS AS PERCEIVED BY THEMSELVES AND THE
READING TEACHER

The reading activities in school particularly the Philippine Informal Reading


Inventory (PHIL-IRI) which determine the reading level of students both in public
and private schools was viewed to entail challenges to teachers and to students
that may affect their reading performance. A list of challenges is provided below,
check ONE answer that would best describe the extent of each challenges (as
expressed in each of the statements) that you have encountered. Each response
