MOTION FOR WITHDRAWAL OF COMPLAINT/REQUEST FOR

ASSISTANCE

Come now the undersigned parties, by themselves, and unto this

Honorable Office most respectfully states:

1. That on July 5, 2021 the undersigned was urged by their co-

employees to go to DOLE-NLRC for alleged consultation.

2. Movant was also directed to sign a document given by DOLE-NLRC

and filled-up by their co-employees.

3. Thereafter, he learned that the same is a complaint against herein

responding company and its employees.

4. That when he requested for a copy of the same, his co-employees

refused to give them a copy thereof.

5. That upon consultation and reflection, the undersigned employee

request that he be excluded as complainant against the responding
parties since his consents and permissions was vitiated and he has no
intention to file any case against the responding parties.
WHEREFORE, premises considered, it is respectfully prayed that the
undersigned movant be excluded as complainant in the hereto docketed
case/s.

Other reliefs just and equitable are also prayed for under the foregoing
premises.
