Was job safety analysis carried out and safety talk with all team members carried out?
Have bridge/engine room watch keeper been informed of intended work?
Have combustible solids/liquids/gases above or adjacent to work been cleared/treated/tested?
Have the appropriate authorities been informed? (If applicable) and is there authorisation from offshore platform/Port
authority been given?
Have bunkering operations ceased (if applicable)?
Is suitable fire fighting equipment readily available?
Has combustible material in adjacent spaces close to the hot work been cleared of any substances that may ignite?
Is there a fire watch keeper appointed and briefed, with suitable fire fighting equipment?
Is the correct PPE being worn by the person involved in welding/burning operation? Welding Goggles / Leather Gauntlets?
Will the operation be under regular observation?
Are there any hazardous/dangerous goods, which may be affected by the work?
Are adjacent openings, hatches intakes and vents screened/closed if applicable)?
Is there due regard for any wiring on remote side of deck or bulkheads?
Is ventilation adequate?
Is lighting adequate for the job?
Has the welding equipment been inspected before use? Hoses Gauges Valves Torch
Are flashback arrestors in place?
Manufacturers manual checked for operational and performance data?
Are all electric components of the equipment inspected and in satisfactory condition?
Is the electric intensity regulated for the welding?
Are welding rods correct for the type of welding to be carried out?
ALL ANSWERS MUST BE AFFIRMATIVE IF APPLICABLE TO THE JOB
