Subject: Complaint Regarding the Invalid Delivery Proof and No Identity Proof of the
Receiver for AWB 930237407054

I, Shellita Chloe Bautista, owner of a DTI & BIR-registered local business, Kabagera BagShop
located in Imus Cavite, would like to file a COMPLAINT against J&T Cotabato Branch located at
Sesami St. Rosario Heights Cotabato city, and request for an immediate investigation regarding
STAFF Insubordination & Connivance with ONLINE SCAMMERS. 

My issue started with a parcel sent via J&T Paliparan, insured for 30k, it has clear instructions
that the parcel should be PICKED UP only at the said branch, of a certain “Angie Cruz”. Nov 3,
we called the branch to request proof that the parcel was picked up - an ID of the said client or
CCTV that indeed it was picked up at the branch. However, the branch failed to give us any
proof that it was picked up, but to our surprise, they admitted that it has been delivered.
Admission was made by their rider, KING LAGUIALAM, that indeed, he delivered it to ANGIE
CRUZ since he knew this “person of interest”. This is where the issue really begins, why was
the packaged delivered and not picked up? NO Identification was shown to the branch by the
receiver, if indeed it was a certain Angie Cruz who received the parcel. This is definitely a case
of false registration and a breach of the company’s SOP with regards to PARCEL HANDLING &
Security. 

I have been a loyal client of J&T for 3 years now and I hope you will extend the courtesy of
investigating this matter, and I demand a FULL CLAIM of 30k from the said branch for
FAILURE to provide proof and mishandling of our PARCEL. Give me proof of who this “Angie
Cruz” is and with complete details, address and pictures and identification, and I will drop all
complaints. 

If not given the proper attention, the next letter you will receive will be coming straight from my
lawyer.
