How to measure vital signs First we need to prepare the tools in advance including
the air pressure above the watch thermometer or stopwatch and PPE (personal
protective equipment). Examination of vital signs, including blood pressure,
respiratory rate, pulse, and body temperature.

Before taking action to measure vital signs we must ask for the patient's consent and
ask the patient's name where the date of birth is in accordance with hospital
regulations

1. First, we check the patient's blood pressure by using blood pressure. Normal
blood pressure is between 90/60 to 120/80.
2. Next, take the temperature measurement by placing the thermometer in the
patient's armpit, wait for 3-5 minutes or wait until there is a sound indicating
the measurement is complete.
3. Then measure the pulse first you press gently on the wrist until you can feel
the pulse, then count the pulse within 1 minute or 60 seconds. Normal pulse
between 60-100 beats a minute
4. After that measure the respiratory rate by looking at the movement of the
chest wall in 1 minute. normally 16 - 24 times per minute
5. Lastly document all actions then say goodbye to the patient.
