Tata Consultancy Services (TCS), sometimes through its contracted partners, would like to collect and share,
as described below in this Release, the following material (“Testimonials”) from you (tick those that
apply):

□ Photographic / Image capture □ Video Case Study / Testimonial

1. With the acceptance of this Release, you grant TCS, its affiliates and other entities that TCS may
designate from time to time, the right to collect, distribute, broadcast, stream, edit, translate or
otherwise disseminate the Testimonials, without any further approval from you, in whole or in part,
throughout the world, in perpetuity, in any and all media now known or hereafter developed.
2. This grant to TCS includes, but is not limited to, the right to use the Testimonials either alone or
together with supporting information, such as your name, voice, photograph, likeness and
biographical data (collectively, "Supporting Information").
3. Personal information included in the recording of the Testimonials, together with the Supporting
Information and the other details however collected with this form, will be processed by TCS
according the privacy notice published on the website https://www.tcs.com/privacy-policy (the
"Privacy Notice") and for the any further purpose specified in this agreement. The applicable lawful
basis for processing the above indicated media is the performance of the contract, namely this
Release; your personal data will be shared, for the same purposes, with third party providers like
events management companies, photo and video productions and technical partners for the
dissemination of the content. Testimonials and relevant Supporting Information will be shared with
other third parties, like social networks and other communication channels for the same purposes.

4. You can request TCS to stop using the Testimonials at any time, in which case the Company will not
use the Testimonials for new purposes as from that moment, but it may continue to appear in
publications already in circulation.

5. You are solely responsible for all content and material provided by you incorporated within your
Testimonials. You will ensure that all third-party material used by you (including but not limited to
presentations, audio and video material, images and brand names) is covered by a permission,
release or license to enable the recording of your Testimonials consistent with the possible usages
set out in Article 1. You ensure to TCS that any content provided by is not defamatory, unlawful
and, to the best of your knowledge, does not violate the rights of any third party.

6. You release TCS from any claims or demands arising from the use of the Testimonials. TCS will not
be responsible for any unauthorized use of your Testimonials.

7. TCS will ensure by all means possible to not make use of the Testimonials in a way that is
defamatory, unlawful, detrimental for or in violation of your rights.

8. You agree that you will not be entitled to compensation, remuneration or royalties in any form for
the usage of the Testimonials.
