LOCAL GUIDELINES ON OJT AND INTERNSHIP PROGRAMS,


SUMMER 2021

To adhere with Memorandum Order no. 7, series of 2021, dated July 31, PUP Unisan
Branch shall comply with the following provisions:

1. The on-the-job training and internship programs SHALL BE via online and/or
correspondence mode.

2. Face-to face reporting/deployment at OJT or internship sites, even in at any PUP


Branch/Campus, is NOT ALLOWED.

3. Alternative activities shall be given such as online trainings, webinars, case analyses
and other projects that will help enhance the knowledge and skills in their field of
specialization.

4. It shall be the duty of the OJT Instructor to provide:

4.2 materials for case study analyses

4.3 enhancement projects to enhance the competencies

5. The target competencies for the OJT/Internship shall be as follows:

5.1 DICT 1- Information Technology Assistance

5.2 DICT 2- Junior Programming

5.3 DOMT 1- General Office Practice

5.4 DOMT 2- Legal Office Procedures

5.5 BSENT 3- Marketing and Salesmanship

6. It shall be the duty of the OJT Instructor as well, to measure objectively the level of
competencies attained by the students during midterm and finals period.

7. Instructional Materials are also necessary, aligned with the outline of proposed
activities for the entire summer classes. IM’s are also subjected for evaluation of the
IM’s Evaluation Committee.

STUDENTS ON THE-JOB TRAINING (OFT)/INTERNSHIP PROGRAM MANUAL

General Guidelines

The OIT/Internship is a hands-on, experiential course of study whereby the students


undergo DIT/internship activity in any agency/company or establishment/ government
and non-government offices that will expose them to administrative and operational
functions, task, work situations and environment related to their chosen program and to
develop the necessary competencies and skills needed in their chosen fields.

Qualification

The following students are eligible to undergo student OIT/Internship:

1. Regular Students-Officially enrolled 3 year and

2. Irregular students-only those with a maximum of 6-unit deficiency will be allowed to


take the OJT/internship, subject to a conditional clause regarding the extension of the
OJT/internship periods to meet the requirements.

Procedures

1. O/T/Internship Orientation is conducted prior to the OJT/internship Period.

2. The OIT/Internship Coordinator assigned in the college/department is responsible for


selecting Agency/Company/ where the students will be trained for their OJT/Internship;
however, students can search their Agency or Company for OJT/internship but subject
to the approval of the OJT/Internship Coordinator, and in accordance with the
institutional guidelines on the selection of Institutions

3. The college or department releases the list of students who met the requirements and
qualified for the OJT/Internship.

4. The students prepare a File Folder/Clear Book (color coded) for submission to the
OJT/Internship Adviser assigned to them at the end of the OJT/Internship Period.

MEMORANDUM OF AGREEMENT

Know All Men By These Presents:

This MEMORANDUM OF AGREEMENT entered into by and between:

The (name of company), a corporation duly created and existing under the
laws of the Republic of the Philippines, with office address at (address), herein
represented by its (position of authorized signatory of the company), (name
of authorized signatory of the company), hereinafter referred to as the
COMPANY;

-And-

The POLYTECHNIC UNIVERSITY OF THE PHILIPPINES, a state university


with principal address at Anonas St., Sta. Mesa, Manila, herein represented by
its Vice President for Academic Affairs, DR. EMANUEL C. DE GUZMAN,
hereinafter referred to as UNIVERSITY.

WITNESSETH:

WHEREAS, the Commission on Higher Education (CHED), on 24 May 2020,


issued CHED Covid Advisory No. 7, wherein it stated that OJT and internship programs
(foreign and local) remain suspended until ECQ, MECQ, GCQ, and MGCQ has been
lifted. On 10 February 2021, the CHED and the Department of Health (DOH) issued
Joint Memorandum Circular Order No. 2021-001 wherein CHED and DOH prioritized
selected health-related degree programs regarded as vital in providing additional
manpower support in the health system, for the gradual reopening of HEI campuses for
limited face-to-face classes and internship.

WHEREAS, pursuant to the aforementioned issuances, the Office of the Vice


President for Academic Affairs, through Memorandum Order No. 2, Series of 2021,
allowed the offering of On-the-Job Training (OJT) and Internship Programs on the 2nd
Semester of SY 2020-2021 by means of online and/or correspondence mode of delivery.

WHEREAS, the UNIVERSITY, has established a professional program for its


students, under the College of ______________, called the On-line Internship Program
for Second Semester 2020-2021. The program is designed to supplement the learnings
in (name of the program) and to train students to become competitive and
technologically innovative professionals through actual exposure to a professional
workplace through on-line platform to address the new normal brought about by COVID-
19;

WHEREAS, the COMPANY supports this program and is willing to give the
students of the UNIVERSITY a course related work assignments and expose them to
actual learning experiences;

NOW THEREFORE, for in consideration of the foregoing premises, the parties


hereby bind themselves to undertake this Agreement under the following terms and
conditions:

1. The duration of the program shall be equivalent to at least 200 hours unless
otherwise agreed upon by the COMPANY and the UNIVERSITY;
2. The COMPANY shall commit to take in the student-intern to undergo Internship
Program consistent with the requirements of the UNIVERSITY;

3. The UNIVERSITY shall be responsible for briefing the student-intern who intend
to conduct practicum exposure in the COMPANY as part of the UNIVERSITYs
curriculum;

4. The UNIVERSITY shall provide the student-intern the basic orientation on work
values, behaviour, and discipline to ensure smooth cooperation with the
COMPANY;
5. The OJT shall be held on-line based on the agreed scheduled between the
supervisor and the student-intern and duly noted by the faculty adviser;

6. The UNIVERSITY shall unilaterally withdraw the student-intern who is found to


misbehave and/or act in defiance to existing standards, rules, regulations of the
COMPANY and impose necessary the UNIVERSITY sanctions against the said
student-intern;

7. The COMPANY shall provide free relevant instruction and training to the intern,
consistent with its policies, rules and regulations. It shall treat the intern in a
professional manner, and shall not subject him/her to tasks and work
assignments that are risky, dangerous or unrelated to the purpose of this
Agreement:

8. There is no employer-employee relationship between the COMPANY and the


intern. As student-intern, he/she shall not be entitled to the
compensation/benefits accorded to an employee;

9. The COMPANY is not obliged to employ the student-intern upon the completion
of the internship. However, the COMPANY, upon consultation with the
UNIVERSITY, may require qualified intern to submit themselves to examinations,
interviews, and submit pertinent documents to support their application;

10. The intern shall be personally responsible for any and all liabilities arising from
gross negligence in the performance of his/her duties and functions while under
internship;

11. The parties shall not divulge any information that it may have access to, and any
such information will only be used for academic purposes;

12. This Agreement shall commence on _________ and shall terminate on


_________ unless earlier terminated for just cause, by either party after thirty (30)
days prior written notice to the other party.

12.1. Further, either party shall have the right to pre-terminate the internship
program if:

a) The student-intern violates the rules and regulations of the COMPANY, or if there
is serious misconduct on the part of the student-intern;
b) The COMPANY does not provide the kind of responsible training as agreed upon;
or
c) There is any violation of the foregoing covenants that will warrant the cancellation
of this Agreement.

PROVIDED that the terminating party shall formally inform the Program
Coordinator or Faculty Adviser of the grievance before any decision to terminate is
made and finalized. The Program Coordinator or Faculty Adviser shall intervene in order
to rectify the situation in the interest of all parties concerned.

12.2. The one year period shall be applicable to cover other prospective student-
interns who may likewise be required to undergo an Internship with the
COMPANY during the affectivity of this Agreement.

12.3. Any renewal of this Agreement shall be through the mutual written consent by
both parties.

13. Both parties shall exert all efforts necessary to achieve the objectives of this
Agreement.

Eatery Eatery is a casual or informal word for a place

to buy and eat prepared food. A diner is one

kind of eatery, and a Chinese restaurant is

another.
COMPANY HISTORY

All the business in the country has its own history. History can tell the how the business
starts, overcome the problems and how able to survive the business and go to
successful state of business that earn profits, create job and employment, giving service
and helping economy of the country. In line of business it have two types of business
the micro- and macro business even if the business is micro it doesn't matter because it
business also that provides needs and gives service to the people.

The history of Belen Leo's store starts with the owner of this store named Mrs Belen
Ferrer. She had a beautiful personalities that attached to her being human. Mrs. Belen
had strong personality and positive mind so don't be surprise to the success of her
business. Mrs. Belen have disable feet every day it can give her a lot of pain her
children are away from her because all of them had their own families elsewhere. With
the reason of that if Mrs. Belen in pain her husband only give helps to her. Everyday
Mrs Belen are very lonely to her house so that she fined way how to resolve her
problem.

Mrs. Ferrer decided to build micro business, store that located at Brgy. Cabulihan
Unisan Quezon (Crossing) with the name of store Belen Leo's Store. The foundation of
the Store is 2019, since 2019 the store serves all the people in that particular area.
Circumstances and difficulties Passover in that business, this store are standing strong
until now and gives service to people.

The Belen Leo's Store are very famous in the Cabulihan Unisan Quezon because of the
high quality of the products that its offer to the customers. All the peopled who works at
the store are very kind, humble and gentle that's why many customers are willing to buy
some products of the store. In the terms of the growth and expansion of this store,
starting 2019 of operation of the store there has only establishment and lack of wide are
but now it had additional two establishment that you can buy any pair of sandals, slipper,
ukay-ukay, school supplies and etc. The growth and expansion of the store really helps
the customer and people around that store.
LOGO

Logo in one business are important, logo that gives distinctive image to the customer
and also to all people. It is the way easy to recognize business by the symbol that
shown at the logo. The logo of the Belen Leo’s store has a 3 colours which are yellow,
sky blue, and black. Owner says that the black colours symbolize midnight which all the
people are in the deep sleep means the silence. The store are very SILENCE in terms
of competing but it is very dangerous if you compete with them. The yellow colour
symbolize the colour of the sun, store that always rise for the new morning, new
opportunities and new life. Store also gives a beautiful morning to the people and
whether the typhoon can’t stop the rise of Store. The sky blue colour represents to the
peace inside the store being even there have competition in line of business peace are
imperishable. The symbol that shown in the logo are the star and the house. The star
symbolize that behind the store there have a people that (represent the star) who
always work hard, dedicated to their work for the success of the store. People behind
the store are the star that never fade, shining Yesterday, Today and tomorrow for the
effulgence of the Store. The little house symbolize the store, store that gives services, it
also symbolize that even its small it can give huge happiness.

SUMMARY REPORT

We all know that the Polytechnic University of the Philippines established the
program for its students. The On Job Training programs aims to develop the knowledge
and skills of the students through the actual exposure to the Professional workplace. As
a trainee of the company that I, perform my On Job training I found myself that I can
handle any problems and circumstances that pass in my job. In the workplace that I
been there I am very happy and thankful to the owner of store because she teach me
some techniques how to deal with customers and how to manage business not for loss
but to earn more and more profits.

In addition the area of my work in that store have the connection to my course.
Marketing the product and service are my job in that particular store. Belen Leo’s store
has a quality product that they can offer to the costumers in addition all the peopled in
that Store are very friendly that’s why many customers are attracted to that Store.

Me as a trainee during the On the Job Training I do all my best to be the assets of the
Store I worked hard to learned more skills and knowledge that I can used it when I apply
in the Company in the future. Dealing with the customers are not very easy so as a
giving a service we all know that the customers are always right. I am very thankful to
the owner of the store and also to the employees of that they gave me more experience,
practicing me how to deal in the customers and how to be a responsible person in every
part of my work even if it is small or big.

I remember the first day of my training I, am nervous not because I don’t know all
what I need to do, I am nervous because I am very excited on the job that the owner
assign on me. The owner give some tips for my job and instruction also I understand all
the owner say and I do all I can for the job that assign in me.

Before I, undergo On the Job training I know that train is the best to all people to be
expert in work, job and life. When one person are undergo in training it is advantage for
him or her. During my OJT I, realize that being sociable, real, responsible, kind, honest
and humble are very important when you are in the workplace it can helps you to deal to
the costumers and have a good and strong relation to them. After On The Job Training I
learned that training are very important to everyone because being train in all things can
Allow you to pass to all the circumstance in work. On the Job Training can gives
Additional skills, techniques and knowledge that are very helpful to all of us in the line of
future Entrepreneurs.

Belen Leo’s Store open starting every Monday to Sunday at 6:00 AM To 8:00 PM. As a
trainee on that workplace I, set my schedule every Monday to Saturday at 12:00PM to
5:00 Pm in the afternoon. That particular date and time of my work schedule are
favorable to me as a student. I am very thankful because the owner of that store are
very understanding to me and to my schedule.

Celebration are very popular this day, now is the time of pandemic there are
many lockdown places in the word. Even all that worst year because of the pandemic
we as a Filipinos cannot stop for that pandemic. Filipinos always find the way to be
happy and to enjoy life. I remember when I was in the workplace in the store. The owner
of the store celebrate her birthday in that place all the employee of the store are very
happy. There have a lots of foods and also having a videooke in the store. All the
people of the store are enjoying the celebration and I am very happy to join their and I
am so thankful also because of the owner warm heart to her peopled.

The difficulties that I had encountered during the OJT are in the line of dealing
with the customers of the store. Other customer are very complicated I cannot
understand what are they talking about or complain about because all I do is to
understand all those customers. There has a scenario that one of the customers
complain that the service that we provide cannot satisfy what he/ she expected to us so
the owner taking action to the customer immediately. The owner talked the customer
and try to understand what he are trying to say. Because that the owner of the store are
very humble the problem are being solved in that day. So this are one of the difficulties
during my OJT.

Even if there are many difficulties during my OJT I am here to solve the problem I
encountered all the best I can. I am not expert but I believe that the difficulties I
encountered are part of the training that I must be known to be have more knowledge
about work that I can used in the future.

Answers:
1. What could be the issue in this case?
The issue of this case is unexpected Promotion of Lina to the government and the
mishandle of performance evaluation that caused conflict between the two parties.

2. Did Lina make the performance evaluation of Imelda at the right time? Defend
your answer.
For me, Lina didn't make the performance evaluation of Emilda in the right time
because Lina don't have didn't know more about the performance evaluation. Lina only
knows previous experience about performance evaluation she didn't know the deep
meaning of the performance evaluation that's why there have a conflict regarding to the
promotion of Lina. For me Lina needs to improve her knowledge about learning
performance evaluation before she go in the other coworkers and set the evaluation to
avoid such conflict in workplace.

3. Was Lina the right person to be promoted as supervisor? Explain your stand.
In the promotion of one person on the company, the company must sure and know
the employee that they can promote. Promotion is one of the great achievement in one
employee so the employee that will be able to promote or to be promoted is the person
that deserves that position. For me Lina is the right person to promote why? Because
she is popular in the terms of being sociable and also having a good performance in the
Work beside, Lina need more time to improve the part of her work to be more
productive
And avoid conflict with coworker. Being popular is not the reason to promote one
employee the best way and right way to promote is if the employee is skilled, full of
knowledge and has professional attitude that not caused any problem. I conclude that if
Lina had all of this character she is liable for the promotion of the company.

My reaction if I experienced that case is the like the Imelda reaction but being a trained
students that undergone to the OJT I will manage my emotion and be humble to

1. Who do you think has the competence to do the evaluation of jobs and
companies pay sale program?
A job evaluation is a systematic way of determining the value/worth of a job in relation to
other jobs in an organization. It tries to make a systematic comparison between jobs to
assess their relative worth for the purpose of establishing a rational pay structure. In
that case the duties of personnel administrator include evaluation of the jobs of the
company's. For me, the personnel administrator has a competence to to the evaluation
of the jobs of the company's. Why? Because evaluation of the job and assessing an
employee's performance are the duties and job of the personnel administrator so the
other people don't ever think that the personnel administrator has a bias side to the
employee. For that the employee that complain and make chaos and make people
angry are needed to accept the fact and the result of evaluation. In the case that says
the the newly hired employee has a higher grade of evaluation, the employee may now
that evaluation are not only look at the people position in the company, evaluation look
to the job description and the administrator have no intention to make such a move that
no connection to their job. The personnel administrator do their job only not in personal
it only for the company that's why I think that that personnel administrator has be able
and competence to do the evaluation of the job.

Insure that the capability of the company in that particular program that they may eliminate are
very helpful and make a valuable results not to create chaos, but to create Unity and stability of
the company. The company before their establish that kind of evaluation they sure that the

Yes, because the employees has a right to evaluate and asses their own job and their
job description. Peer and Self Assessments Often, peer assessments and self-

assessments employees have the right to underline what they think their performance is,
and why certain metrics may be misleading. Peer assessments and self-assessments
are useful in capturing this data: Peer assessments : members of a group evaluate and
appraise the performance of their fellow group members. Self-assessments: in self-
assessments, individuals assess and evaluate their own behavior and job performance.
