Understanding
Nursing Process
The nursing process is a key factor in patient
survival and in the maintenance,
rehabilitation, and prevention aspects of
health care.

In nursing care is divided into 5 stages,


namely the stage of assessment, diagnosis,
interpretation, implementation, and
evaluation.
The nursing process aims to:
- Using troubleshooting methods
- Using standards for nursing practice
- Obtaining a standard and appropriate
method, rational (logical) and systematic
(sorted, neat).
- Obtain a method that can be used in all
situations.
- Have high-quality nursing care outcomes.

Assessment
Stage
Nursing assessment is aimed at the client's response
to health problems related to the fulfillment of basic
human needs.

In the process of collecting data, nurses must be able


to classify data. This aims to find out more accurate
data so that the next stage of nursing care does not
occur errors. The following are two types of data in the
assessment process.
1. Subjective data
2. Objective data

Data collection methods in the assessment phase are


as follows.
1. Communication or interaction
2. Observation or observation
3. Physical examunation or measurement

DIAGNOSIS STAGE
All nursing diagnoses must be supported by data,
which according to NANDA is defined as a
"definition of characteristics". The definition of
these characteristics is called "signs and
symptoms". A sign is something that can be
observed and a symptom is something that is felt
by the client.

Determination of the diagnosis will affect the


actions that will be given to the patient. To prevent
things that are not desirable, then there are
several stages that must be mastered by nurses in
formulating a diagnosis. The following are the
stages of nursing diagnosis.

Planning includes developing
strategies to prevent, reduce or
correct problems identified by
nursing diagnoses (Lyer, Taptic
& Bernocchi-Losey 996).

The steps taken during the


planning stage are as follows.

the results (outcomes)

Implementation
Phase
The purpose of implementation is to assist clients in achieving
the goals that have been set, which include health promotion,
disease prevention, health recovery and facilitating coping.
Nursing action planning will be carried out properly, if the client
wishes to participate in the implementation of nursing actions.

EVALUATION
STAGE
Evaluation is an intellectual
action to complete the nursing
process which indicates how far
the nursing diagnosis, action
plan, and implementation have
been successfully achieved.

The purpose of evaluation is to


see the client's ability to achieve
goals.

evaluation are as follows.

due to illness.
Advantages and Disadvantages of Home Health Care

Profit Loss
1. Home settings can provide more comfort for clients in undergoing individual treatment.
2. Many clients prefer to be treated at home.
3. Assessment of environmental factors that support health can be more complete because it can be
observed directly so that it can be directly considered about what services are suitable for clients
financially, etc.
4. It is easier to study lifestyle and family norms.
5. Participation of family members can be well facilitated.
6. Family members may be more eager to accept and learn things that can improve or support their
health because they are applicable and in accordance with conditions at home.
7. Can shorten the length of stay in the hospital so that treatment costs can decrease

Loss
1. Travel costs for nurses or home health care providers are expensive.
2. Less efficient than joint nursing practice or client visits to the ward.
3. Distraction for example: children and the sound of the TV is difficult to avoid.
4. The safety of nurses in carrying out nursing care is not so well maintained.

Description Of Duties Of Implementing Nurses In Icumentation Room

1. Maintain the cleanliness of the treatment room and its environment.


2. Receiving new patients according to the procedure and in letter ii, I inform you that
my child has a condition that applies.
3. Maintain nursing and medical equipment so that it is always ready for use.
4. Conduct nursing assessments and determine nursing diagnoses according to the
limits of their authority.
5. Develop nursing plans according to their abilities.
6. Develop a nursing plan for the patient according to the needs of his ability
7. Train / help the patient to do motion exercises.
8. Perform emergency actions to patients (including high fever, collapse, bleeding,
poisoning, respiratory arrest and cardiac arrest), in accordance with the applicable
procedures and then immediately report the actions taken to the inpatient room
doctor/doctor on duty.
9. Carry out evaluation of nursing actions according to the limits of their abilities.
10. Observing the patient's condition, then taking appropriate action based on the
