Subject: Dengue Fever Awareness

For the past two weeks, multiple instances of dengue fever have been detected in our schools.
In four separate classes, nine instances of dengue fever have been documented. The risk of
spreading is significant, and both teachers and parents are becoming concerned. High fever of
104 degrees Fahrenheit (40 degrees Celsius), headaches, muscle, bone, or joint pain, nausea,
vomiting, discomfort behind the eyes, swollen glands, and rash are all signs of dengue fever.
Children who are suspected of having a viral illness will be sent home to be treated and
diagnosed by their family physician.

The major techniques for preventing the development of dengue fever are still avoiding
mosquito bites and decreasing the mosquito population. Encourage your students to apply
insect repellent and to wear protective clothing like long sleeves and pants.

For now, stay healthy. Take some time to observe your students to make sure they are free
from this illness that is going around. When children feel well and are healthy, they enjoy
school more and learn better. Thank you for your attention to this matter.

Interprofessional Team Meeting
Meeting Minutes
November 28, 2016

Opening
The Interprofessional Healthcare team meeting was called to order at 2:00 P.M on November 28,
2016 in The Cranium Focus by Diane Birch.

Present
Diane Birch, Physician/ Facilitator
Olivia Roberts, Advance Practice Registered Nurse (NP)
Ashley Williams, Clinical Nurse Navigator (RN)
Dahlia Smith, Medical Social Worker
Amelie Cordua, Health Promoter/ Community Health Worker
Allanah Crystal Abello, Registered Dietician/ Diabetic Educator

Approval of Agenda
The agenda was unanimously approved as distributed

Approval of Minutes
The minutes of the previous meeting were unanimously approved as distributed

Discussion
To begin, the team discussed Mr. John Smith's modifications and brainstormed ideas for programs
that might be beneficial to Mr. Smith. Second, each team member had provided his or her ideas and
plans for Mr. Smith's home options, Diabetes education class, and up-to-date medication list for his
virtual hallucinations. Everyone agreed on the following plans:

Finally, the team had come out with few suggestions of relevant plans for Mr. John Smith. Each
member of the team would choose what day would they go for home visit, reconcile with his
medication list and also educating him about his hyperglycemia which is the side effects of this
medications (e.g., Zyprexa and Haldol).

Action Items:
 The team has finalized the suggestions and plan of care.
 All suggestions were listed in the Microsoft word for everyone’s reference
 The meeting has been recorded and link was shared via YouTube group

Adjournment
The meeting was adjourned at 4:00 PM as scheduled.
