1. Assess the newborn’s condition.
PLANNING
2. Gather the necessary supplies and bring to the bedside stand
or overbed table
3. Perform hand hygiene and put on clean gloves.
4. Identify the newborn.
IMPLEMENTATION:
Newborn weight
5. Place a lining on the weighing scale and zero out the weighing
scale.
6. Remove the clothing and diaper of the newborn.
7. Place the newborn on the center of scale tray. Make sure that all
body parts are on the weighing scale.
8. Weigh the newborn on a weighing scale making sure that
nothing interferes, or nothing is affecting the weighing scale.
9. Wait for a couple of seconds for the weighing scale to stabilize
and note the weight of the newborn.
Newborn Head Circumference
10. Pace the newborn on the bassinet or flat surface on a table or
the prep area, keeping warm and protected.
11. Use a tape measure, position the tape just above the
eyebrows, above the ears or helix of the pinna and around the
biggest part of the head
12. Pull tape snuggly to compress the hair.
13. Keep measuring tape snug (not too tight, not too loose) and note
the head circumference.
Newborn Chest Circumference
14. Using a tape measure, measure at the mark of the nipples
15. Make sure that the tape measure is snugly fitted (not too tight
and not too loose) and note the chest circumference
Newborn Abdominal Circumference
16. Using a tape measure, measure on top of the umbilical stump
17. Make sure that the tape measure is snug (not tight, not loose)
and note the abdominal circumference
Newborn length
18. Place newborn on a side lying position.
19. Using a tape measure, measure from head to the heel of the toe
and straighten the legs on the knee part.
20. One person should hold the newborn in place while the other
completes the measurements and note the length.
21. If using infantometer, the newborn’s back in the center of the
length board, and is lying straight, shoulders & buttocks are flat.
22. Both legs should be fully extended & the toes should be pointing
upward with feet flat against the foot piece and note the
measurement.
23. Remove the newborn in the infantometer.
24. Do newborn care and comb hair.
25. Remove clean gloves. Discard on appropriate receptacle.
26. Put on newborn’s clothing, mittens, socks, and diaper. Swaddle
the newborn and give it to the mother.
27. Perform hand hygiene.

28. Evaluate the following:
a. Comfort of the newborn
b. Breathing and Skin color
DOCUMENTATION
29. Document the following:
a. Length and weight
b. Head, chest, and abdominal circumference
c. Note comfort of the newborn
