Do Medical Hand Washing, Apply Alcohol then Dry
Don Working Gloves
When baby is placed in the abdomen, dry the baby for at least 30 seconds starting
from the face and head going down to the trunk and extremities. Perform as well
a quick check for breathing.
Remove the wet cloth and keep the baby in prone position on the mother’s chest
or abdomen. It will not only provide warm but also it will stimulate the bond
between the mother and the baby. It will also promote the success of
breastfeeding.
Cover the baby with a dry linen and put a bonnet to prevent heat loss. Place the
identification bond of the baby as per hospital policy.

Receive and Clean the Baby after Unang Yakap
- Remove excess blood from the Vernex. Do not remove the vernex
Birthweight: with the newborn naked, place in a weighing scale. State the normal
birthweight of a newborn
- Baby is brought to the weighing area well wrapped for Birth Weight with
the Pediatrician/Staff nurse assigned
- Take note of the weight
Vital Signs of the Baby
- Temperature : Initially Rectal then Axillary thereafter.
- Cardiac Rate: Warm and put on Chest for a minute and record
- Respiratory Rate: Place the stethoscope over the chest or abdomen, count
rise and fall and observe for retraction and record
Administer Eye Ointment from inner to outer canthus of the eyes.
TAKING THE ANTHROPOMETRIC MEASUREMENTS
Head Circumference:
- Take the Tape Measure
- Lower border is near to the eyebrow and upper tip of the ears.
- Support the baby’s head.
- Position the tape measure facing you
- Measure and Read (state normal range)
Arm circumference: measure the mid upper arm circumference.
Chest Circumference
- Turn on side
- Slowly slide the tape measure until the back of the chest, then stretch.
- Place in line with the nipple then read
Abdominal Circumference
- Position nest to the Umbilical cord the read
- Remove tape measure downward
Length
- Tip should be in line of the heel
- Turn the baby in the side
- Measure the above feet
- Stretch a little the extremities up to the Vertex of the Head
- Use thumb as an indicator. Back to supine and read
Administer vitamin K 0.1 ml IM according to hospital policy
Administer Hepatitis B vaccine 0.5 ml IM according to hospital policy.
Put clothes on the baby.
Do after care.
Remove gloves
Perform hand hygiene
Monitor vital signs every 15 minutes for the first hour, every 30 minutes for the
second hour then hourly.
Document.
