Prior To Patient’s Transfer To The Delivery Room
Ensured that mother is in her position of choice while in labor.
Asked mother if she wishes to eat/drink.
Communicated with the mother – informed her of progress of labor, gave
reassurance and encouragement
Preparing For Delivery
Check temperature in DR area to be 25-28 degrees Celsius; check for air draft.
Removed all jewelry.
Wash hands thoroughly observing the WHO 1-2-3-4-5 procedure.
Arrange materials/supplies in a linear sequence:
• 2 sterile gloves
• 2 dry linens
• Bonnet
• Plastic clamp
• Blade
• Kidney basins
• Tape measure (as per institution)
• 2 syringes (1ml syringe) for hepatitis B vaccine and vitamin K
• Erythromycin ointment
• Cotton balls
• Stethoscope (for physical examination of the newborn)
Identify the mother and explain the procedure to be done.
Asked mother if she is comfortable in the semi-upright position (the default
position of delivery table).
Clean the perineum with antiseptic solution.
Wash hands.
Put on sterile gloves aseptically. If the same healthcare provider handles the
perineum and cord, put on 2 pairs of sterile gloves aseptically.
At the time of Delivery
Encourage woman to push as desired.
Call out time of birth and sex of baby.
Inform the mother of the outcome. `
First 30 seconds
Place the baby on a clean, dry cloth/towel on the mother’s abdomen.
Thoroughly dry the baby for at least 30 seconds, starting from the face and head,
going down to the trunk and extremities.
1-3 minutes
Remove the wet cloth.
Place baby in skin-to-skin contact on the mother’s abdomen.
Cover the baby with a clean, dry cloth/towel.
Cover baby’s head with a bonnet.
Give IM oxytocin within one minute of baby’s birth after the health personnel
exclude a 2nd baby.
Remove the 1st set of gloves
Palpate umbilical cord to check for pulsations.
After pulsations stopped, clamp cord using the plastic clamp or cord tie at 2 cm
from base.
Place the instrument clamp/ hemostat instrument 5cm from the base.
Cut near plastic clamp at least 1 cm (not midway).
Check baby’s color and breathing. Do an APGAR score within 1 minute and 5
minutes after delivery.
Performed the remaining steps of the AMTSL (Active management of the third
stage of labour):
• Waited for strong uterine contractions then applied controlled cord traction
and counter traction on the uterus, continuing until placenta was delivered.
• Massaged the uterus until it is firm.
Inspected the lower vagina and perineum for lacerations/tears and repaired
lacerations/tears, as necessary.
Examined the placenta for completeness and abnormalities.
Cleaned the mother: flushed perineum and applied perineal pad/napkin/cloth.
Checked that mother was comfortable, uterus contracted
Disposed of the placenta in a leak-proof container or plastic bag.
Advise mother to maintain skin-to skin contact. Baby should be prone on mother’s
chest/in between the breasts with head turned to one side.
15-90 minutes
Advise mother to observe for feeding cues and cited examples of feeding cues.
Support mother; instruct her on positioning and attachment.
Wait for FULL BREASTFEED to be complete.
After a complete breastfeed, administer eye ointment (first), do a thorough
physical examination, then did Vit. K, hepatitis B and BCG injections
(simultaneously explain purpose of each intervention). Get the anthropometric
measurement of the baby: weight, length, HC, CC, abdominal circumference,
MAC, thigh circumference.
Advise OPTIONAL / DELAY bathing of baby (and was able to explain the rationale).
Advise breastfeeding per demand and about Danger Signs for early referral.
In the first hour: check baby’s breathing and color and vital signs monitoring.
In the second hour: checked mother-baby dyad every 30 minutes to 1 hour.
Complete all RECORDS: administer eye ointment, Vit. K, hepatitis B and BCG,
Anthropometric measurements. Document the procedure.
Wash hands.
