IMMEDIATE NEWBORN CARE

- activities that will prevent occurrence of untoward effect in the newborn including
hypothermia, infection, and bleeding.

Purposes:
- To prevent bleeding from the cord
- To prevent infection of the cord and eyes
- To prevent hypothermia

I. PROCEDURE 5 4 3 2 1
1. Disinfects and prepares the crib before the delivery of the
newborn.
2. Prepares the two (2) droplights.
3. Assemble all equipment.
- Right handed: all equip on the right side, working area on left side
- Place the 1ST layette over warmer
- ARRANGE:
a. dress (TOP)
b. diaper
c. bonnet
d. booties and mittens
e. 2ND LAYETTE
-Binder: to restrain newborn knees applying
meds
4. Performs medical handwashing then, disinfects hand using
sanitizer.
5. Prepares Kidney basin as waste receptacle.
6. Prepares the hypotray: inner (sterile)

a. 4 Cotton Balls (2 dry; 2 soaked with alcohol)


b. Tape measure – for anthropometric measurement
c. Eye Ointment
d. Vitamin K Ampule
e. Tuberculin Syringe
f. Thermometer
g. Comb

7. Prepare medications (Terramycin opthalmic ointment; Vitamin K)

A. CREDE’S PROPHYLAXIS
- Application of ophthalmic ointment to newborn eyes to prevent neonatal
blindness or Ophthalmia Neonatorum caused by gonorrheal infection as
the newborn passes through the mother’s birth canal.
- Check manufacturing date, expiration date and name of drug

B. VITAMIN K
- Generic name: phytomenadione
- Full- term Neonates: 0.1 cc/IM@left thigh (vastus Lateralis)
- Preterm Neonates: 0.05cc/IM @left thigh (vastus lateralis)
> Disinfect the head with ACB
> Dry CB wrap around head, dot facing away
> Withdraw 0.1 cc, aspirate air, fish hook technique

C. Hepatitis B Vaccine
- 0.5cc/IM/ Right thigh (vastus lateralis)
8. Washes hands and don a pair of working gloves (protect self from
mother discharges).
9. Upon receiving the newborn, weighs the baby(note),
a. place baby on the layette and check the ID band on foot part;
b. performs quick check of Head-to-toe assessment (gender, bruises and
deformities)
c. turn newborn to side, to check spinal alignment; provides warmth
10. Takes Vital Signs for 1 full minute (RR, CR, rectal Temp)
a. RECTAL TEMP
- to check for patency
- support both feet, slightly fold
- insert thermo in twisting motion slowly, inserting only the tip
- note result, place after in waste receptacle

b. CR:
- warm diaphragm, count 1 full min based in wall clock

c. RR:
- place hand over chest or diaphragm
11. Takes the Anthropometric Measurements accurately and record
it. (Head is bigger than the chest, chest bigger than abm)
A. HEAD CIRCUMFERENCE
- lower border align with eyebrow and tip of ears

B. CHEST CIRCUMFERENCE
- slowly turn to sides while lowering down the tape measurements
- place aligns with nipples

C. ABDOMINAL CIRCUMFERENCE
- place next to umbilical cord

D.LENGTH
- turn to side
- position from heel of the feet up to vertex of the head
- slowly place back the baby, check for the result
12. Restrain the upper and lower
extremities.

13. Applies Crede’s Prophylaxis properly from inner to outer


canthus
a. Check 3 times (manu & exp date, name)
b. discards the first drop in kidney basin
c. lower down cheek exposing lower lid
d. sticks out medicine, come contact w/ lower lid, placing from
inner to outer canthus
14. Gets 1 sterile cotton ball with alcohol and disinfects the site for
intramuscular injection (inner to outer)
15. Injects the vitamin K correctly into the vastus lateralis, and
applies light pressure using dry CB.
a. Vastus lateralis: In between anterior and lateral of the thigh
b. Place palm on the knee of the baby to lock
c. Perform cushion fashion; Inject 90%, support the hub, aspirate,
slowly introduce drug.
d. remove injection and press
16. Releases the restraints from the upper and lower extremities.
17. Takes the vital signs (RR,CR,Temp:axillary).
18. Cleanses the newborn using a towel and comb.
- performing while supporting the head of the baby
19. Dress the Newborn:
a. Put on the newborn’s sterile gown
b. Insert a clean bonnet
b. Put on diaper
b. Put on mittens and booties (remove extra thread)
c. Change the newborn’s layette
20. Swaddle the baby.

21. Does the after care. Soaks instruments for 30 minutes in a


disinfectant solution then rinses and dries the instruments.
22. Document accurately.

23. Monitors the vital signs every 15 minutes until stable. Then,
every 30 minutes X 4 takes.
