1. Gave analysis for table 3.7 (Main Source of Water Supply)

The family of Garcia’s main water source is the commercial water provided by the Prime Water. The
cost depends on their monthly consumption. Their estimated monthly consumption is Php300.00.
Moreover, the Garcia family owns a manual water pump (groundwater) where they get water to
mainly water their plants and sometimes to wash their clothes. The groundwater source lessens their
expenses. Although they live in an urban area, sometimes they experience a loss of water connection
from the Prime Water so they use the groundwater as an alternative. The groundwater is free but also
depends on the availability of water (e,g. el Nino). Additionally, they buy their drinking water at a
water refilling station.

2. Gave analysis for table 3.8 (Distance of the Septic Tank from the Source of Water Supply
(in meters if ground water)

The septic tank is located 10 meters away from the groundwater supply. According to
recommendations by the United States Environmental Protection Agency, as well as the Department
of Housing and Urban Development, a septic tank should be at least 50 feet away from a well that is
used for drinking water. The groundwater is safe since it is only used for watering the plants, cleaning,
and sometimes washing clothes. Moreover, the septic tanks of neighbouring houses do not affect the
water source of the family because they are far from each other.

3. Gave analysis for table 3.9 (Toilet Facility)

In the toilet facility of the Garcia family, they have a water-sealed toilet, sewer and a septic tank that
is maintained in a good condition. They also use a pail and dipper system for bathing. The toilet facility
is exclusively used by the household. All of the family members share one toilet room as well as the
bathing room.

4. Gave analysis for physical assessment (Patient Glea)

The table above shows Glea Laureen’s physical assessment conducted on October 26, 2021. Glea is 26
years old and was born on March 6, 1996. For her general survey, her height is 149 cm, weight is 40
kg (BMI: 18). Glea is considered as mild underweight. BMI less than 18.5 falls within the underweight
range. During the physical assessment, vital signs were also taken. Her body temperature is 36.5;
blood pressure 110/80; pulse rate 78 bmp; respiratory rate 15 breaths per minute. Her mental status
is alert since she responds quickly to any questions asked and as per observation. She doesn’t have
any allergies to food and medication according to her mother. Upon assessment, her skin was warm,
no lesions and abnormalities were detected. Her airway is patent, and her breathing pattern is normal.
For her cardiopulmonary, breath sounds are clear, and her cardiac is in a regular rhythm. Her capillary
refill is 2 seconds and her jugular neck veins are not visible. As per her pupil, it is round, equal in size,
and reactive to light. For her eyes, she was diagnosed with myopia, and she is using an eyeglass (175
oculus dexter; 105 oculus sinister). Her ears, nose, mouth, teeth, throat, neck, cardiovascular,
abdomen, musculoskeletal ROM, leg are all normal.

5. Gave analysis for physical assessment (Patient Lhara)

The table above shows Lhara Garcia’s physical assessment conducted on October 26, 2021. Lhara is
30 years old and was born on December 12, 1990. For her general survey, her height is 153 cm, weight
is 59 kg (BMI: 25.2). Based on the result of her BMI, she is considered overweight. BMI within 25.0 to
<30 falls within the overweight range. During the physical assessment, vital signs were also taken. Her
body temperature is 36.7 that falls into normal; blood pressure 90/60; pulse rate 108 bmp; respiratory
rate 17 breaths per minute. During her physical assessment, she verbalized that she felt nauseous and
dizzy because she is 22 weeks pregnant. Her mental status is alert since she responds quickly to any
questions asked and as per observation. She doesn’t have any allergies to food and medication
according to her mother. Upon assessment, her skin was warm, no lesions, discolorations and
abnormalities were detected. Her airway is patent, and her breathing pattern is normal. Skin should
be assessed in order to see abnormalities that could be a sign of underlying conditions like
dehydration. For her cardiopulmonary, breath sounds are clear, and her cardiac is in a regular rhythm.
For her capillary refill, the color returned to the original color in 1 second and it is considered normal.
Capillary refill should be less than 2 seconds. Her jugular neck veins are not visible. As per her pupil, it
is round, equal in size, and reactive to light. For her eyes, ears, nose, mouth, teeth (wearing braces),
throat, neck, cardiovascular, abdomen, musculoskeletal ROM, leg assessment are all normal.
