Good day!

I am respectfully informing your kind office of my application for a nursing employment, preferably
for HRH Deployment. I’m a graduate of Bachelor of Science in Nursing at the Western Mindanao
State University last March 30, 2005. I’m a registered nurse qualified by the Professional Regulation
Commission dated February 16, 2006. I am 36 years of age, single and in good health. I have gained
a considerable skills, knowledge and attitude regarding nursing practices. Recently, I finished my
employment as a hemodialysis nurse with Diaverum AB Branch, Ministry of Health, Kingdom of Saudi
Arabia from February 14, 2017 until February 14, 2019.

Currently, I'm working as a staff nurse at Zamboanga Dialysis Center, Veterans Avenue, since
October 07, 2019.

I have accomplished 3 units from my Graduate School for Masters of Arts in Nursing at Western
Mindanao State University last second semester of 2009- 2010. I served as a volunteer nurse with
Brent Hospital and Colleges Incorporated last February 25, 2010 until May 10, 2010 and reapplied at
the same institution since September 27, 2010 until April 15, 2012 as a renal care nurse.

I have completed my volunteer- training for nurses for three months, last March 06, 2006 to June 6,
2006 in Pediatric unit and serve as a Nurse- I staff in Medical- Surgical and OB-GYN unit at the,
former, Western Mindanao Medical Center, last June 07, 2006 to December 15, 2007. Subsequently,
I had undergone training, last September 01, 2008 to September 30, 2008, on hemodialysis nursing
at the Renal Center in Zamboanga City Medical Center and qualified as a Certified Renal Nurse under
RENAP as a hemodialysis staff nurse, of the same institution, last December 16, 2007 to October 31,
2009.
