SMALL SCALE MINING PERMITS (SSMP)

Mandatory Requirements for Small Scale Mining Permit:

1. Application Form ✓
2. For corporation/ partnership/ association/ cooperative: SEC/CDA-certified true copies ✓
of Certificate of Registration, Articles of Incorporation/ Partnership/Association and By-
Laws

3. Location Map/Sketch Plan (1:50,000 NAMRIA topographic map) showing ✓

coordinates/boundaries (in tabulated form) with major environmental features/other
projects (prepared, sealed and signed by a deputized Geodetic Engineer)

4. Work Program (prepared, sealed and signed by a licensed Mining Engineer or 2-years
Geologist)
5. Proof of technical competence in the form of: ✓
a. CV and track records in exploration/ environmental management.
b. Sworn statement of the technical person(s) who will undertake the implementation
of the Work Programs or employment contract/proof of availed service(s)
6. Proof of financial capability: ✓
a. For an individual:
i. Sworn Statement of Assets and Liabilities
ii. ITR for the preceding 3 yrs.
iii. Credit lines/bank guarantees/deposits
b. For corporation/ partnership/ association/ cooperative:
i. Latest audited Financial Statements
ii. Annual Report for the preceding year
iii. Credit lines/bank guarantees/deposits
7. Proof of payment of Filing Fee to the MGB/P/C Mining Regulatory Board

8. Other supporting papers as the MGB/P/C Mining Regulatory Board may require or ✓
the applicant may submit

Additional Requirements after Acceptance of Application

1. Environmental Compliance Certificate ✓
2. Environmental Protection and Enhancement Program ✓
3. Approved Survey Plan/Application for Order of Survey ✓

Filing of Application
1. By the applicant personally or
2. Through its duly authorized representative

Venue of Application:
1. Areas within mineral reservations – MGB
2. Areas outside mineral reservations - P/C Mining Regulatory Board concerned

Procedure:
1. Evaluation that terms and conditions and all pertinent requirements are in order
2. Subject area is cleared from any conflict
3. Approval and issuance of SSMP

4. Payment of fees
5. Within 15 days from receipt of written notice, Permit Holder shall cause the registration of the permit
6. If 2 months after filing, applicant is awaiting approval and mandatory requirements have been
complied
a. Applicant shall file Affidavit of Attestation of full compliance with all pertinent requirements
b. Application shall be deemed approve
c. Within 5 working days from receipt of affidavit, MGB shall issue SSMP for registration and
release

Mandatory Requirements for Renewal:

1. Application for renewal filed at MGB, cc: regional office
2. Payment of Renewal Fee
3. Proofs of payment of occupation fee, excise tax and royalty
4. Financial Report
a. For individuals - Latest ITR
b. For corporation, partnership, association or cooperative - Audited FS
5. 2-year Utilization Work Program duly prepared, signed and sealed by a licensed Mining Engineer or
Geologist
6. If 1 months after filing, applicant is awaiting approval and mandatory requirements have been
complied
a. Applicant shall file Affidavit of Attestation of full compliance with all pertinent requirements
b. Application shall be deemed approve
c. Within 5 working days from receipt of affidavit, MGB shall issue SSMP for registration and
release

Additional Requirements after Acceptance of Application for Renewal:

1. Environmental Protection and Enhancement Program
2. Certification of compliance of SSMP issued by the regional office
