Contractor HSE Plan Guideline

HEALTH, SAFETY AND ENVIRONMENT (HSE) PLAN GUIDELINE

A. HSE Plan Requirements

 The CONTRACTOR shall prepare and submit to the COMPANY for approval a job
specific HSE Plan prior to commencing any work or service which describes the
HSE activities that the CONTRACTOR will perform to meet COMPANY policy above
and achieve excellent HSE performance.

 The CONTRACTOR shall demonstrate the existence and compliance with a HSE
Management System to the COMPANY's satisfaction for the Work or the SERVICES
are provided solely in the CONTRACTOR's office or site.

 The CONTRACTOR shall prepare the HSE Plan and send to COMPANY for approval
within thirty (30) days of award of Contract or before mobilisation of the
CONTRACTOR's PERSONNEL or EQUIPMENT to Site, whichever is the earliest.

 The review and approval of the HSE Plan by the COMPANY shall not relieve the
CONTRACTOR from its obligation to maintain a safe system of work, provide a
safe place of work, safe EQUIPMENT and compliance with all relevant laws,
regulations and Statutes.

 Services will not commence until an approved HSE plan is in effect.

B. HSE Plan Contents

 Each activity of the HSE Plan shall specify the following:

a) Quality/quantity standards;

b) Time frame/frequencies to be achieved;

c) Responsibilities.

 The activities of the HSE plan shall be measurable for auditing purposes.

 The extent and detail of the HSE plan will vary depending on the scope of the
Work or the SERVICES, their complexity and the anticipated hazards.

The HSE Plan shall address the following items but not be limited to:

(1) Management Commitment

 CONTRACTOR shall identify the COMPANY and CONTRACTOR HSE Policy


placement and communication plan at the workplace during contract
execution in a language that is easily understood by the workers.

 CONTRACTOR’s shall develop program (if any) on management


activities as example below.

i. Endorsement of HSE policies.

ii. Monthly site visits as minimum or prior mobilization, 2 times


during the contract, and demobilization preparation.

iii. Lead safety meetings on monthly basis.

iv. Employee recognition and feedback.

v. Participation in audits and follow up on monthly as minimum or


prior mobilization, 2 times during the contract, and
demobilization preparation.

vi. Accident investigation when required.

The program shall be planned in regular basis as per proposal by


CONTRACTOR.

(2) Organization and Responsibilities

The CONTRACTOR organization for the work and HSE responsibilities of each
PERSONNEL shall be clearly defined in the HSE Plan as follows:

 The CONTRACTOR shall appoint PERSONNEL for ultimate HSE


responsibility and duties.

 The CONTRACTOR shall establish organization chart.

 The CONTRACTOR shall describe competencies for each position in the


organization.

 Should any specialized skill or ad hoc PERSONNEL required in


performing the service or work shall be also included in the
organization.

(3) PERSONNEL Selection, Competence and Training

The CONTRACTOR shall defined and stated the mechanism of personnel


selection (if any), competency and training matrix which fulfill:

 The required qualifications relevant with the duties.

 The nominated HSE representatives and their responsibilities.

 Competencies, training, and certification required relevant to the


duties.

 Induction plan to all CONTRACTOR AND SUBCONTRACTOR’s


PERSONNEL with regards to HSE procedures, Organization as well as
the services performed by CONTRACTOR.

(4) HSE Objectives and Targets

HSE Objectives and targets for the works or services performed shall be
clearly stated and person responsible to achieve them.

The objectives and targets shall cover the followings:

 Man-hour performed

 Lagging indicator i.e. number of safety incident (Fatality, Lost Work


Days Case, Restricted Work Days Case, Medical Treatment Case,
First Aid Case), spill, property damage, and others.

 Leading indicator i.e. audit and inspection program, pre job meeting,
daily permit meeting, HSE monthly meeting, and others initiated by
the CONTRACTOR.

All targets shall be clearly stated to be reviewed by the end of the contract
whether it is achieved or not.

(5) SUBCONTRACTOR Management (If Any)

CONTRACTOR shall describe a method on:

 Selection method and process.

 Expectations/requirements communication between CONTRACTOR &


SUBCONTRACTOR.

 HSE Performance monitoring on complying with regulations and


standard.

 Auditing and maintaining past SUBCONTRACTOR data and records.

(6) External Professional Assistance (If Any)

Identification of external professional assistance (in country or outside the


country) as required such as emergency services, security forces, shall
cover as follows:

 The CONTRACTOR shall identify a means of communication for


external professional assistance.

 Authorization and implementation of the procedures shall be clearly


understood and tested.

(7) Communication

 The CONTRACTOR shall provide means of communication.

 The CONTRACTOR shall appoint person responsible for maintaining


communication, understood both in normal situation and in an
emergency.

 The CONTRACTOR shall provide adequate backup communication


means are available.

 The CONTRACTOR shall carry out regular communication test plan on


weekly basis.

(8) Safety Meetings/ Employee Involvement

The CONTRACTOR shall conduct HSE meeting to meet the HSE objectives.
The HSE meeting shall be planned as a minimum as follows:

 Pre-mobilization meeting.

 Pre-work commencement meeting.

 Pre job meeting for all activities.

 Daily permit to work meeting for all activities.

 Monthly HSE meeting lead by management.

 Pre work phase change or any other meetings that the CONTRACTOR
feels are appropriate to maintain a safe working environment.

 All regular meetings are to be minute with a copy provided to the


COMPANY Representative.

 "Tool box" or brief safety meetings during the job need not be
minute but their occurrence should be documented in daily reports.

 Where practical, all PERSONNEL, CONTRACTOR and SUB-


CONTRACTORs shall attend all safety meetings.

 Attendance list and reasons of PERSONNEL absences.

(9) HSE Promotion

The CONTRACTOR shall plan the HSE promotion to communicate and


improve awareness of HSE matters to all members of workforce; the mean
shall be one or combined of the followings means:

 Notice boards,

 Coaching,

 Counseling,

 Newsletter,

 Intranet, etc.

(10) HSE Orientation Program

The CONTRACTOR shall conduct an orientation program to establish


awareness, accountability, and delivery performance or the followings
PERSONNEL:

 New employees hired.

 Visitors to the Company premises.

 Transferred employees.

 On-The-Job orientation, etc.

 A follow up program shall be in place to ensure understanding of the


employee.

(11) Matrix Competencies

Contractor shall identify training and competency of each personnel to meet


specific requirements of critical activities. Training matrix shall be reviewed
to assure all personnel working at Eni Indonesia operational area are
suitably trained and competent.

If any incompleteness of the training as per required in the matrix, training


plan shall be prepared accordingly.

(12) Inspections

The CONTRACTOR shall carry out inspection program to ensure that the
workplace is safe, EQUIPMENT is maintained and suitable for the Work or
the Services, hazards are identified and that PERSONNEL are working in a
safe manner and no damage to the environment.

Inspections shall include where appropriate on the followings:

 Pre-mobilization of the PERSONNEL and EQUIPMENT to the Company


premises.

 Routine maintenance checks of EQUIPMENT during the contract


period at least on weekly basis.

 Vessel or barge and vehicles to ensure compliance with relevant


standards and relevant regulations prior and during contract period
at least on monthly basis.

 Housekeeping, walkthrough, workplace hazards, emergency


response, etc. at least on weekly basis.

(13) Hazard Identification

 The CONTRACTOR shall define what system(s) will be used to


identify, assess and manage hazards.

 Housekeeping, walkthrough, workplace hazards, emergency


response, etc. at least on weekly basis

 The CONTRACTOR shall, where possible, shall be prepare JSA prior


to mobilization and reviewed on site prior to the job.

 The CONTRACTOR shall plan hazard identification and assessment


systems such as HAZOPs, HAZIDs or other similar systems that are
intended to be used.

(14) Control of Work Procedure

Company procedure shall be referred by contractor during the


commencement of work at Eni Indonesia operational area. If any specific
requirement is not addressed by company procedure, Contractor shall
define the reference of control during conducting of work. Bridging
document shall be prepared by the Contractor and approved accordingly by
both company and contractor.

(15) Emergency Response

 The CONTRACTOR shall establish CONTRACTOR’s Emergency


Response Plan (include subcontractor if available).

 The CONTRACTOR shall carry out regular exercise at least on


biweekly basis to ensure that they are adequate for the Service, both
on site and off site.

(16) Reviews/Audits

The CONTRACTOR shall plan the reviews/audit program to ensure compliance


with CONTRACTOR, COMPANY and other standards, regulations, rules and
HSE Plans, as appropriate.

 Audits and Reviews to be carried out by the CONTRACTOR in regular


bases during delivering the services.

 The CONTRACTOR shall submit the audit report and follow up.

 The CONTRACTOR shall submit the close out status.

(17) Documentation and Reporting (Including Incident Investigation)

The CONTRACTOR shall keep the followings documentation on site:

 HSE manuals,

 Operating procedures,

 Maintenance records,

 HSE plan,

 Minutes of meetings,

 Incident/accident reports,

 Inspection reports.

 Etc.

The CONTRACTOR shall conduct incident investigation and report it


accordingly to COMPANY as part of commitment to follow up the post
incident management. The reporting and investigation method of incident
shall be under approval COMPANY (as per COMPANY procedure).

(18) Health Surveillance

 The CONTRACTOR shall have a list of medical for pre-employment.

 CONTRACTOR shall carry out regular medical examination program


and fitness to work for employees.

 Contractor shall have medical evacuation plan including list of


hospital,

 The CONTRACTOR shall have list of programs for the identification,


control and monitoring of occupational health hazards e.g. stress,
long hours of work, sunburn, noise, radiation, asbestos, chemicals
exposure or other respiratory monitoring programs, etc.

 Contractor shall describe the provision of health management


system plan including provision of medical support, personnel and
facilities

(19) Waste Management Plan

Contractor shall describe waste management plant which cover the


following, but not limited to:

 Waste segregation and collection

 Waste labelling and documentation

 Waste transportation and disposal

Any waste handling mechanism shall comply with government of Indonesia


regulation, and specific permit shall be by authorized body

If contractor, alongside with the agreement with the company, will used
company waste management system, approval is required prior work
commencement
