As required in the CONTRACT OF SERVICES and TERMS OF REFERENCE (TOR), the undersigned SECOND
PARTY for the month of August 2021, has:

1) Rendered advisory services to the majority of the members of the Sangguniang Panlalawigan
on their powers and functions under the Local Government Code;

2) Provided advisory services to the Office of the Governor thru the GLLO on laws and legislative
matters;

3) Discussed with the members Memorandum Circular 126 s. 2006 of the Office of the President
regarding filling of vacancies in the Sangguniang Bayan caused by members who do not belong to any
political party

4) Provided inputs on Executive Order No. 138 of the Office of the President entitled FULL
DEVOLUTION OF CERTAIN FUNCTIONS OF THE EXECUTIVE BRANCH TO LOCAL GOVERNMENTS,
CREATION OF A COMMITTEE ON DEVOLUTION, AND FOR OTHER PURPOSES

5) Attended weekly meetings of the majority members of the Sangguniang Panlalawigan;

6) Performed other duties and functions which were requested by the GLLO and the SP in the discharge
of their functions.
