ELEMENTS OF
2. A clear end goal
INTERDISCIPLINARY
TEAM 3. Effective communication

4. Teamwork

5. Shared leadership

17
PATIENT-FOCUSED PRACTICE

The needs of the patient are prioritized

by a successful interdisciplinary team.
Every decision and aspect of care is
discussed with the patient and their
family or caregiver. This ensures that
not only are the patient's requirements
met, but that the patient also feels
heard and respected.
04
A CLEAR END GOAL

The end objective should be the focus

of all aspects of the care plans. It could
be for the purpose of improving a
patient's function, restoring their usual
quality of life, or even saving their life.
Knowing what the end goal is can help
define the team's purpose, point
everyone in the right direction, and
04
inform the treatments they consider
and use.
EFFECTIVE COMMUNICATION

Each team member must be able to

communicate specific patient information to
other team members. This necessitates the use
of verbal skills to clearly describe symptoms
and treatments, nonverbal skills to assist
listeners in receiving your information, and
written skills to communicate in charts and
correspondence. In order to set care goals that
fit the patient's needs, contact with the patient
04 and their family is also required.
TEAMWORK

Each member of the team must recognize

and respect their colleagues' strengths, but
they must also be willing to take on tasks to
assist or add to the roles of others. This
necessitates that everyone recognizes the
scope of their expertise and areas of overlap
with other disciplines, respects the opinions
of others, and recognizes that they may learn
04 from other fields.
SHARED LEADERSHIP

The role of the leader in an interdisciplinary

approach might be a flexible identification.
Frequently, the team recognizes that the
member with the most expertise or
understanding in a circumstance should take
the lead in managing the problem.
