How to Apply
‘A Step By Step Guide for Completing Your Application’
If you are an individual applicant:
If you are a facility, skip to page 20

Step One

● Visit www.dataflowgroup.com
● Click on the QCHP logo
● Select the option that applies to you
● Login using the email ID and password you received at your registered email
Step Two

● If you are a new user, click the ‘New user? Register here’ button on the login page
and fill the requested fields
● If you forgot your password, click the ‘Forgot Password’ button on the login page -
after which you will receive your new password at your registered email
Step Three

● Upon login, read and agree to the disclaimer - then click ‘Save and Next’
● On the Package Details page, select your Application Type, Application Category and
Application Sub Category
● Click ‘Save and Next’

Options under Application Type include:

❖ New Case
❖ Retrospective
❖ Additional
❖ Report Transfer
● If you select the ‘Additional’ option under Application Type - you will be required to
enter your previous QCHP Reference Number in order to request an additional PSV
check(s).
● If you select the ‘Report Transfer’ option under Application Type - you will be required
to provide the following:
● Barcode
● Previous DataFlow Group report
Step Four

● On the Personal Details page, enter the required information then click ‘Save and
Next’ to proceed
Step Five

● On the Education Details page, enter the required information then click ‘Save and
Next’ to proceed

Note: You may add multiple components - if required. However, you will incur additional
charges for each. If you do not have any additional documents for verification, state
accordingly in the comment section in order to proceed.
Step Six

● On the Health License Details page, enter the required information then click ‘Save
and Next’ to proceed

Note: You may add multiple components - if required. However, you will incur additional
charges for each. If you do not have any additional documents for verification, state
accordingly in the comment section in order to proceed.
Step Seven

● On the Certificate of Good Standing page, enter the required information then click
‘Save and Next’ to proceed

Note: You may add multiple components - if required. However, you will incur additional
charges for each.
Step Eight

● On the Employment Details page, enter the required information then click ‘Save and
Next’ to proceed

Note: You may add multiple components - if required. However, you will incur additional
charges for each. If you do not have any additional documents for verification, state
accordingly in the comment section in order to proceed.
Step Nine

● On the Logbook Details page, enter the required information then click ‘Save and
Next’ to proceed

Note: You may add multiple components - if required. However, you will incur additional
charges for each.
Step Ten

● On the Letter of Authorization page - download, sign and upload a scanned copy of
your Letter of Authorization
● Click ‘Save and Next’ to proceed
Step Eleven

● Carefully review your application, then click ‘Save and Next’ to proceed to the payment
gateway
Step Twelve

● Carefully review the auto-calculated amount


● Once you settle your payment, you will be redirected to the portal with a thank you
message and will receive a payment confirmation email containing your payment
receipt
● Once verification is successfully completed by the DataFlow Group, you will receive
notifications at your registered email
If you are a facility:

Step One

● Visit www.dataflowgroup.com
● Click on the QCHP logo
● Select the option that applies to you

Note: Once your application(s) is approved by your assigned DataFlow Group associate and
your account is fully activated, login using the email ID and password received at your
registered email
Step Two

● If you are a new user, click the ‘New user? Register here’ button on the login page
and fill the requested fields

● If you forgot your password, click the ‘Forgot Password’ button on the login page -
after which you will receive your new password at your registered email
Step Three

● Upon login, read and agree to the disclaimer - then click ‘Save and Next’
Step Four

● If you are a new user - select the ‘New Application’ checkbox, fill the listed fields and
click ‘Save and Next’
● Otherwise - select the ‘Edit Application’ checkbox, edit the application(s) and click
‘Save and Next’
Step Five

● On the Package Details page, select your Application Type, Application Category and
Application Sub-Category
● Click ‘Save and Next’
Step Six

● On the Personal Details page, enter the required information then click ‘Save and
Next’ to proceed

Note: You will be charged based on the package selected and additional components
entered
Step Seven

● On the Applicant Document Details page, enter the required component information
and documents as per the selected package then click on the ‘Save and Add More‘
button to save the details to the grid
Step Eight

● Saving the details will save the application(s) to the Applicant Cart Details page

● To initiate multiple applications, click on the ‘Add More Application’ button


● To make a payment via credit card, select the application(s) - after which you will be
redirected to the payment summary page
Step Nine

● Once you have selected the application(s) via the credit card option, you will be
redirected to the ‘Employer Payment Details’ page to confirm your package details
● To complete the payment, click on the ‘Make Payment’ button

● Once you settle your payment, you will be redirected to the portal with a thank you
message and will receive a payment confirmation email containing your payment
receipt(s)
● Once verification is successfully completed by the DataFlow Group, you will receive
notifications at your registered email
Insufficiency Cases

In the case that an application(s) is marked as ‘Insufficiency’ by the DataFlow Group - you will
receive a message at your registered email.

Login to your account and select the ‘Edit Application’ button to review and fulfil the missing
requirements.
