solemnly affirm and declare on oath as under:

1. That we all permanent residents of J&K State.

2. That we are partners of firm M/s SHEIKH-UL-ALAM ENTERPRISE

With its Principal place of business at Hasab Complex,
near Shivpora Crossing, Srinagar.

3. That we intend to open the current with J&K Bank B/o:

B.B.Cantt Sriangar in the name of the above said firm.

4. That we the above named partners of the firm neither jointly

nor severally are maintaining current account with any other
bank.

5. That neither the above mentioned firm nor any of its partners
are indebted to any bank or financial institution.

6. That we will abide by all the rules and regulations of the

Bank governing allowing me to run such current account.

7. Hence this affidavit.

Colony, Sanath Nagar, Srinagar, being the general secretary of Society namely
Social Justice and Development Foundation Kashmir (SJDFT) having its
registered office (Registration applied for) at 57-Al-Farooq Colony, Sanath Nagar,
Rawalpora, Srinagar do hereby solemnly affirm and declare on oath as under:

1/ That is am the permanent resident of J&K State.

2/ That I intend to open the provisional current account with The J&K Bank
Ltd. B/O Jawaharnagar, Srinagar in the name of the above said Society till the
registration is obtained whereupon the provisional account shall be converted
into permenant account.

3/ That the said account will be operated under the hand and seal of both
General Secretary and Treasurer of the society, as laid down in the rules and
regulations and “minutes of the first meeting of the society on 5 th of December
2009” and a copy of which is attached herewith.

4/ That neither the above mentioned society nor any of its members are
indebted to any bank or financial institution.

5/ That the deponent will abide by all the rules and regulations of the Bank
governing allowing me to run such current account.
