Safety Alert — Incident Recall fp EHVPD - COA L Jan. 2018 oN Serious Injuries Incident During Shifting (AC/DC) Panels Introduction: One of the contractor technician and other person trom public were involved in an unfortunate accident, which result in serious jajuries, lost time injury& property damaged, Briefed Incident deseription: The incident occurred on January 30, 2018, 4:30 PM at substation project site. While shifting AC/DC Panel (#6) from outside the building to L1OKV GIS room using a crane, the panel eyebolt was broken during lifting, and consequently, the pane] was unbalanced, lowered and swung resulted in the injury of two persons (one technician & one audience from the public). Action: It’s very crucial to ensure all the risk factors related to the activity had been fully addressed and accounted /communicated with related workers prior starting the task, especially critical ones, To avoid simitar incidents in the future, you are urgently requested to emphasize the following procedures include but are not limited to the following points » é Page 1 of 2 THIS DOCUMENT IS FOR INTERNAL USE ONLY Safety Alert — Incident Recall Ssec'S'star sarery EHVPD - COA be Jan. 2018 ™N + Close supervision always required (the Project Engineer, the Safety Manager! Engineer or Safety Representative) during the critical lifting task, * Security guard of the project site shall secure the project site to prevent the entry of any external audience to the work vicinity. + Toolbox talk (TBT) should be conducted to the involved lifting team. * The lifting plan should be provided before commencing any lifting activity. ‘+ The Lifting Plan and Risk Assessment should be approved by the consultant engineer (5) / Project Management or his representative before the start of| execution of the job. ‘©The internal work permit (PTW) should be signed by the authorized level such as the approved issuer / receiver Engineers, The lifting equipment operators must be competent with supporting documents ‘+ The validity of the accredited third-party certifications! TUVs should be maintained for lifting equipment and its operators as well ‘+ A competent certified “Rigger” should be available at the site whenever lifting activities are required. ‘+ The material inspection shall be conducted by a competent person before the transporting or lifting process * The inspection of the lifting equipment & lifting gears should be accomplished by a competent person prior starting the work (e.g. safety reps. of the site, the operator of the crane, the "Rigger”), * The necessity of securing and barricading the lifting work vicinity to prevent anyone to be present under lifting load, and the permission should be issued to start lifting only after evacuation of unwanted persons from the place. © The disciplinary actions & appropriate penalties should be applied in case of failure to follow or implement safety instructions.
