revoked.

IMPORTANT NOTE:

The Unit Head concerned shall ensure the following: a) that the subject personnel make a proper turn-over of functions,
responsibilities and accountabilities; b) proper fidelity bonding/insurance coverage is secured for the personnel in case of need;
and c) inclusion to/deletion from IT and other pertinent systems access due to the change in the function/position of the
personnel.
