 IAC#1 was having abnormal sound. PTW to be issued to TMD.

IAC#1, its
AOP supply isolated, its outlet valve closed.
 ADP#1 dew point running poor. ADP#1 isolated from mechanical side, in
view to observe improvement in dew point. TMD has already communicated
to put only two out of three ADP in service.
 ADP-3 display not showing temperature ,attended by C&I.
 LWC issued to BMD for APH hopper inspection. All four hoppers found
filled with ash. Informed to Shri Umesh Gautam (DGM-O).
 IAC#3 was tripping on high current, attended by EMD.
 Leakage oil pump taken in service to empty leakage oil tank. Three empty
drums filled. Informed to TMD to provide more drums to empty. Kindly
follow up.
 Fire protection  Gate valve on TG floor is malfunctioning while checking Fire
protection 1. TMD and C&I on job. Also Shutoff valve is closing only upto
95% when FP is operated from MOT room.

 RC Feeder- C I/L gate actuator is free. Informed & notified to C&I.

(105221299). C&I will take permit after cancellation of Mill CD & feeder C
permit.
 Feeder D permit cancelled by BMD, informed to C&I to take permit for
modification in feeder logic.
 CT Fan- 4.4 PTW cancelled by OFS. Supply normalized.
 PM of LDO PR. PUMP 1 issued to AHM. Supply isolated.
 ESP Pass #B PTW issued to AHM. ESP Pass B isolated, Gate supply, Field
supply, ACP supply isolated and module locked. Earthing done.
Cross PTW issued to AHM for internal inspection of ESP pass B. All
supplies were already  isolated in previous shift.
 ADP 1 temperature not increasing in “Regeneration mode”. In R mode
temperature is rising only 40 deg. Notification raised to C&I. (105220819 dt

 In ADP 2, whenever mode changes from RD mode to CD mode, frequently 1101/1104
faulty alarm appears. Notification raised to C&I.( 105220826 dt 11.11.21)
 PM PTW of CT Fan 3.3 issued to OFS. Supply isolated and riser valve closed.
 Mill#3CD PM PTW along with confined space PTW issued to BMD for inspection.
Also both feeders and SAFs PTW issued to BMD.  Main breaker racked out and
locked, CAD 1C, CAD1D E/ isolated and simulated. SAF C, SAF-D E/supply
isolated. PA gen gate air supply isolated. Both feeders i/l and o/l gates closed and
feeders put in local.
 Mill AB B&S Pressure is low while both pumps are i/s. PTW pending to BMD (PTW
cannot be printed). Mill AB Both B&S pumps supply kept isolated and EPB
pressed.
 Joint checking done by BMD and C&I on S-52 & S-54. Communication recorded
by BMD for limit setting to be done by C&I.
 Communication recorded by BMD, SD14 jointly check done with C&I on 5.11.21,
found ok from BMD side, limit to be set by C&I.
 Air drier 1&3 dew point reported poor as per Chemistry. -0.68/-2.56. Please
follow up 
 pH at deaerator is 9.8 as checked by Chemistry.
 CD 58 passing. Checked by C&I found OK. Informed and Notified to

 CW outlet valve throttled to 20%.
 BFP A DE (Y) vibration hunting, Informed and notified to C&I.105194489. LWC
issued to C&I
 UCB AC compressor 1 NTS PTW issued to TMD. PTW pending

opportunity ADP-1 to be kept as stand by.

 Generator hydrogen coolers and exciter coolers isolated. Hydrogen booster
pump stopped.
 SH SD5/6/7/8 manual downstream closed.
 SD9 common manual valve closed.
 RH SD14 manual valve closed.
 ERM/CRM/hopper heater ON/Filed off.
 AB3 Atomising steam valve stopper rod welding broken. Attended by BMD
 At 06:00 hrs on 2.11.2021, SADC switched from Auto to manual. Significant
difference observed in WB DP transmitter 1 and 2. Notification raised to C&I.

 SD 07 having demand /Feed back mismatch issue. Notified to C&I 105172972. 
SD 07 will be attended after Insulation work.
 Feeder E center Feed pipe coal leakage, Notified to BMD (105174669)
 CAD 3E Showing close limit in running condition, Module made off, Notified to
C&I (105174245), C&I Removed Close Limit wire from Local, CAD 3E is in open
condition in Local, to be closed manually during mill shutdown.
 PT Plant Air Blower A having oil lkg. PTW issued to OFS after isolation.
 Mill CD grease pump A has to be removed as per BMD request. Cable connection
to be removed on communication by BMD. Notified and Informed to EMD.
Supply isolated. PTW to be collected by EMD. Greasing sequence bypassed by
C&I. Connection removed by EMD. Informed to BMD.
 CW-4B got decoupled at 18:11hrs on 07.10.2021, its current came down to 60A,
Main Breaker and discharge v/v supply isolated. Informed to EMD(permit issued)
and C&I for cable disconnection. Electrical Connections removed. C&I
connections removed. OFS Permit pending. 
 WB and LRSB done. WB- 4,6,14,35, tripped on overload. Notified to
EMD(105164492) and LRSB 66 having flange leakage. Notified to
BMD(105164502)
  Poor vacuum maintaining in condenser despite both vacuum pumps in service.
Air ingress suspected. Notified to TMD 105156854.
 CLCW A/B/C Gland Leakage from DE & NDE side,

 Mill EF "E" side coal lkg from main body near hot air chamber found. Notified to
BMD 105152641.
 Feeder F SHUT OFF GATE OPEN limit simulation and Closed limit wire was taken
out by C&I. Notified to C&I 105148399.
 Gland leakage increased from low point drain manual valve. Notification raised to
BMD. (105134911 dt 25.9.21).
 ESP field C1 tripping on DC voltage low. EMD communicated that Electrical
system is OK. Notified to AHM(105122052).
 Pin hole water lkg from CLCW common discharge header in trench, causing filling
up of the trench. Notified to TMD (105118800)
 GT-3 winding temp hunting on desk panel, notified to EMD(105044701).
 Field  A7,C1 U/V and ALI high. Field D7 ALI High High. notified to

Shutdown Defects:

 H2 consumption reported high. Checked by H2 Leak detector

in local. H2 leakage .Observed near hydrogen gas cooler4.

it during shut down in depressurized condition

Condenser Flood Test Protocol

Unit-3, Condenser flood test was carried out on dates 13Nov, 2021. Following leakages were
observed during test:-

1. MS 106 manual drain valve gland leakage.

2. HRH-102 manual valve gland leakage.
3. CRH NRV drain root valve gland leakage.
4. LP flash tank common drain header.
5. 5 CEP-B cooling line leakages.

All the above leakages were attended during test and found ok.
