Electronic submission instructions

This document explains the process and requirements to complete and submit a
Quality Assurance and Mechanical Report for Symbia E / S / T, Symbia Intevo
and e.cam.

Additionally, the mechanical contractor of record will have access to the


electronic Mechanical Report for completion of work and signature. The
mechanical contractor is responsible for submitting an electronic report to
Siemens, and to follow country specific processes. A paper copy is included with
each System Operating Manual and must be completed, signed and left on site.

The Siemens installer is required to fill out the electronic Quality Assurance
Report and his portion, if any, of the Mechanical Report. The installer is required
to submit both electronic documents, and follow their country specific process.

All Reports must be completed fully. There are drop down and check boxes for
various tasks. If a task or option is not part of system startup, “N/A” option must
be selected. Some task boxes require manual entry of “N/A”.

The Reports can be downloaded from the CB-DOC’s link located on the Siemens
Service Intranet, under Installation.

Mechanical Installation Electronic
Report Instructions

1.) Complete the Site Information section.

2.) Scope of Installation Report section: A drop down menu is indicated by


the ◄ symbol; select the system type. Enter the serial and part number of
the system in the appropriate sections.

3.) Measurement Tools section: Enter all calibrated tools used during
installation; “N/A” any unused section.

4.) Mechanical Installation Checklist section: The installation is divided into


16 Steps. Select “N/A” if it does not apply.

5.) Installation Corrective Action section: Following Steps 1 through 16, for
Mechanical Phase, if any corrections are performed, “Yes” must be
selected, and you must indicate which step it corresponds to, and describe
the corrective action that was taken.

6.) Parts Replaced section: Following Steps 1 through 16, for Mechanical
Phase, if any parts are replaced, “Yes” must be selected, and you must
indicate which step it corresponds to, and describe what part number was
replaced.

7.) Mechanical Installation Instruction section: All procedures are available


in drop down menus. Select all Mechanical procedures that apply. Use
“N/A” for any procedure that did not apply. Below is an example of a
procedure pull down list.

8.) Alignment Measurements section: For Symbia S / T and Symbia Intevo


only. Use the pull down boxes to enter data. See example below.

9.) Deviation from Alignment Measurements section: Report any
deviations from Installation procedure.

10.) Calibration Offsets section: Enter or “N/A” offset values from


Diagnostics.

11.) Protective Ground Measurements section: Enter results when


applicable or “N/A” if not applicable.

12.) Signature section: Print, sign, or add digital signature to document.

Instructions for completing the Electronic System


Startup Quality Assurance Report

1.) Complete the Site Information section.

2.) System type section: A drop down menu is indicated by the ◄ symbol;
select the system type. Enter serial number and network information. See
example below.

3.) Additional Workstations section: This section is provided for standalone


workstations shipped with the system.

4.) SPECT Camera Verifications section: Check “Pass” box when test for
Pedestal and Tuning is successful.

5.) Intrinsic Calibration section: Enter values after successful completion.

6.) Intrinsic Verification section: After successful verification, check the
“Pass” box.

7.) Center the Source section: For Symbia E and e.cam only. Enter system
values manually.

8.) COR Calibration section: For Symbia E and e.cam only. Enter results for
single head system only. If you have a dual head system, check the “N/A”
box.

9.) MHR Calibration sections: There are 3 MHR TOMO angles: 180, 90 and
76. Four collimator sets can be entered for each angle. The ◄ symbol
indicates a pull down menu. Select the collimator type that applies to the
TOMO angle, and enter corresponding data. In the example below, only
LEHR and MEDLP collimators are part of the customer’s system. Note
that the other 2 collimator selections are “N/A”.

box.

13.) Additional Verifications section: Check all that applies.

14.) AQC Verification section: For Symbia S, Symbia T and Symbia Intevo
only. Complete if option is present.

15.) CT verifications section: For Symbia T and Symbia Intevo only.

16.) NM CTFOV Calibration section: For Symbia T & Symbia Intevo only.
There are three NM CTFOV sections: 180, 90 and 76. Four collimator
sets can be entered for each angle. The ◄ symbol indicates a pull down
menu. Select the collimator type that applies to the CTFOV angle, and
enter corresponding data. In the below example, only LEHR collimator is
part of the customer’s system. Note that the other 3 collimator selections
are “N/A”.

17.) System Backup section: Check box to indicate a backup was performed.

18.) Installation Corrective Action section: For Commissioning Phase, if any


corrections are performed, “Yes” must be selected, and you must describe
the corrective action that was taken.

19.) Parts Replaced section: For Commissioning Phase, if any parts are
replaced, “Yes” must be selected and you must enter part number and
description of part replaced.

20.) Installation Instructions section: In this section, select the drop down
menu to “N/A” or choose the Quality procedures used for the
commissioning phase. Below example indicates the procedure for Symbia
S / T with IQ.SPECT.
