By signing this contract, I ___________________________ agree to the following terms:

1. I agree to reserve and rent the garment is hereby described as follows:

The rental period is for three (3) days only, including the day when it was taken. I understand that I
am responsible for the return of the rented garment within this period. I must notify Violet’s Gown
Rental if I am going to extend the rental time period before the rental time is up.

2. If I fail to return the rented garment within the allotted time period, I understand that I will be
charged a penalty of Fifty Pesos (P50.00) on a per day basis. This charge will continue until the
gown is returned.

3. Upon choosing my rented garment, I will pay a non-refundable reservation fee equivalent to
25% of the rental fee in order to reserve the garment. The said non-refundable reservation fee
does not form part of the rental fee. If after the reservation made and prior to the taking of the
garment as agreed upon by me and Violet’s Gown Rental, I am no longer interested in renting the
garment, my said non-refundable reservation fee is forfeited in favor of Violet’s Gown Rental.

4. Upon taking of the garment, I will pay in full the rental fee as may be determined by Violet’s
Gown Rental. Further, I will make a deposit equivalent to Two Hundred Pesos or 25% of the rental
fee, whichever is higher. The said deposit is refundable to me after my return of the rented
garment less of any penalties for late return or damages that may be assessed by Violet’s Gown
Rental upon my return of the said rented garment.

5. Rented garments will be returned in the same condition that it was given to me. Any rips, tears
or damage to the rented garment will result in a hold of the deposit until the rented garment is
repaired at my expense. The expense of the repair shall be deducted from the refundable deposit
posted by me. In case my refundable deposit is insufficient to cover the said expenses, I will pay
Violet’s Gown Rental of the excess. Should the damage be so severe that the rented garment
cannot be cleaned or repaired to its original state, I bind myself to pay the full retail price of the
rented garment.

6. I will also deposit one (1) valid ID to Violet’s Gown Rental upon taking of the rented garment and
the same shall be returned only upon return of the rented garment at the condition when the same
was taken by me.

7. I bind myself to take good care of the rented garment. I will not spray any perfume on it nor
cause any damage to it.

8. I understand that this contract is legal and binding and I accept all conditions and terms of this
agreement.

(Violet’s Gown Rental)
 
1. Rental Agreement

o The length of each rental begins when you receive the dress (determined through tracking or USPS delivery

confirmation). The length of rental is 6 days for all dresses on our site. Should you need additional time, each
additional day will be charged at a rate of $10 (ten) per day. Dresses must be postmarked on the last day of
rental. To ensure delivery back to Ballroom Dress Rental it is advisable that you send the dress using a tracking
service (UPS) or delivery confirmation through USPS and insure the dress for its full value, requiring a
signature. If you are unsure of the value of the dress, it will be provided to you. If for any reason the dress is
not returned to Ballroom Dress Rental you will be charged the full retail value of the dress.  You are
responsible for the dress until it arrives back safely at Ballroom Dress Rental.  All returns should be mailed to
Ballroom Dress Rental, 124 Andrien Road, Glen Mills, PA  19342.

2. Deposit

o After choosing your dress, you will be required to provide a 25% non-refundable deposit in order to reserve

the dress.

o The Rental Fee does not include the deposit. The deposit shall be equal to 25% of the Rental Fee. Once you

have chosen a dress, a 25% refundable deposit will be required to reserve the dress. The deposit shall be
returned to you upon the return of the dress in its original condition to Ballroom Dress Rental.

3. Try-On Service

o You may try-on any of our dresses prior to rental to ensure proper fit and satisfaction. You can choose one

dress or several to try-on. You will be charged $20 (twenty)  for shipping and handling. Try-ons must be
postmarked no later than two days after it reaches your residence. You must place a deposit on each dress
that is sent to you for try- on. Once returned the deposit will either be returned to you or applied towards the
cost of your rental. All dresses must be returned in their original condition, unworn and unused to avoid being
charged the full rental fee.  Dresses for Sale must be returned unused, unworn and in their original condition
to avoid being charged the full price of the dress.

4. Late Fees
 o Failure to return your dress (along with any accessories) postmarked by the Return Date will result in a Late

Fee of $10 per day for the first (5) days.

5. Option to Purchase

o Should you decide you would like to purchase your dress after rental, the Rental Fee will be deducted from the

purchase price. Please contact us for additional information.

6. Failure to Return Fee

o Beginning on the 6th day, the Failure to Return Fee will be equal to the full amount of the original Rental Fee

and such amount shall be charged each day the dress is not returned, not to exceed the full retail value of the
dress. The Failure to Return Fee will be charged to your credit card account.

7. Cancellation

o Should you cancel your reservation with less than 21 days notice, you will be charged an additional 25% of the

Rental Fee. Any reservation canceled within 14 days of the Rental Start Date shall be charged the full Rental
Fee.

8. Alteration and Damages

o All dresses must be returned to Ballroom Dress Rental in their original condition. Any alterations must be

undone prior to return to Ballroom Dress Rental. Damages caused by alterations will be deducted from your
deposit. All dresses must be returned in their original condition.  Any damages incurred while the dress is in
your possession, such as rips, tears, staining etc. will be charged to the credit card provided to Ballroom Dress
Rental.  You are responsible to pay for all necessary repairs as well as any lost rental or other fees that are a
result of such damage.

9. Use of Tanning Products

o The use of any permanent staining tanning products is prohibited. As with any other stains, if dress(es) is

returned with tanning stains, damages will be assessed up to the full retail value of the dress.

10. Disclaimer

o Ballroom Dress Rental does not make any representations or warranties as to the suitability or merchantability

of any dress. Ballroom Dress Rental is not responsible for any wardrobe malfunction and shall not have any
liability for damages incurred or arising out of the rental or use of any of our merchandise. All dresses are
rented and accepted in their “as-is” condition.

11. Dry Cleaning

o Dry cleaning is included in the cost of the rental. No additional cleaning fees shall be charged unless the dress

is returned with stain damage. Any dress which is damaged as a result of cleaning will be charged at its full
retail value.
